摘要
目的评价外伤性无虹膜的无晶状体眼带虹膜隔人工晶状体(BDI)植入术的临床效果。方法收集2013年6月至2018年7月本院收治的外伤性晶状体病变合并虹膜缺失50例(50眼)的临床资料,进行回顾性分析。患者均接受Ⅱ期带虹膜隔的人工晶状体植入术治疗,随访6~12个月,分析治疗效果。结果患者术后6个月视力较术前明显提高(t=1.358,P=0.000);术后有3例原有的目眩和畏光症状仍存在,术后6个月基本消失,其余47例术后原有的目眩、流泪及畏光症状术后明显减轻或完全消失。角膜内皮细胞密度术后6个月与术前比较,差异无统计学意义(t=-1.518,P=0.127);术前和术后6个月眼压比较,差异无统计学意义(t=-1.497,P=0.107)。无眼内炎、角膜失代偿、视网膜脱离或人工晶状体偏位、倾斜等并发症发生。结论外伤性无虹膜无晶状体眼采用带虹膜隔人工晶状体植入术安全有效。
Objective To evaluate the efficacy of implantation of intraocular lens with iris diaphragm for the treatment of traumatic aphakia with aniridia.Methods The data of 50 eyes of 50 patients with traumatic aphakia with aniridia from Jun.2013 to Jul.2018 in our hospital were analyzed retrospectively.All cases received secondary surgery of implantation of intraocular lens with iris diaphragm.The followed up time was 6-12 months.Results The visual acuity was significantly improved at 6 months after operation(t=1.358,P=0.000).Three cases with symptoms of dizziness and photophobia were not improved,and the symptoms disappeared at 6 months after operation.Total 47 cases with symptoms of dizziness,lacrimation and photophobia before operation were significantly reduced or completely disappeared at 6 months after operation.The difference in the density of corneal endothelial cells was not statistically significant between 6 months after surgery and before surgery(t=-1.518,P=0.127).The difference in intraocular pressure between preoperatively and postoperatively was not statistically significant(t=-1.497,P=0.107).There were no complication such as endophthalmitis,corneal decompensation,retinal detachment or displacement of IOL.Conclusion Implantation of intraocular lens with iris diaphragm is effective and safe for the treatment of traumatic aphakia and aniridia.
