摘要
目的比较FP7型与FP8型Ahmed青光眼阀(AGV)植入术治疗成人难治性青光眼的效果。方法回顾性分析2015年5月至2019年2月40例成人难治性青光眼的临床资料。患者随机分为两组,25例(25眼)行FP7 AGV植入术,15例(15眼)行FP8 AGV植入术。术后随访12个月。对比两组术前、术后眼压、视力、成功率、并发症及术后用药情况。结果两组术后各时间点平均眼压明显低于术前(P<0.05)。术后1天,FP7组眼压下降低于FP8组(t=-4.761,P<0.01),余时间点两组间眼压差异均无统计学意义(P>0.05);两组术后各时间点抗青光眼用药数量较术前均明显减少,差异均有统计学意义(P<0.05);术后各时间点两组平均抗青光眼药物数量差异无统计学意义(P>0.05)。两组术后12个月手术成功率、视力提高率和并发症发生率比较,差异均无统计学意义(P>0.05)。结论FP7和FP8 AGV植入术在降低眼压、保护视力、并发症和依赖青光眼药物量等方面效果相似,成功率也相当。在成人结膜下空间不足的情况下使用FP8 AGV是一个安全、有效的选择。
Objective To compare the clinical efficacy between FP7 and FP8 Ahmed glaucoma valve(AGV)implantation in the treatment of adult refractory glaucoma.Methods The data of 40 cases of adult refractory glaucoma from May 2015 to Feb.2019 were retrospectively analysed.All cases were divided into two groups randomly.Group A,25 eyes of 25 patients,underwent FP7 AGV implantation,and group B,15 eye of 15 patients,underwent FP8 AGV implantations.The fallow-up time was 12 months.The intraocular pressure,visual acuity,success rate,complications and postoperative medication before and after surgery were compared between the two groups.Results The average intraocular pressure at each time point after surgery were significantly lower in the two groups than those before surgery(P<0.05).The intraocular pressure of the FP7 group was lower than that of the FP8 group at 1 day after surgery(t=-4.762,P<0.01),but there was no significant difference between the two groups at other time points(P>0.05).The number of anti-glaucoma drugs in both groups was significantly reduced after surgery compared with that before surgery,and the difference was statistically significant(P<0.05).The difference in the average number of anti-glaucoma drugs at each postoperative time were not statistically significant between the two groups(P>0.05).There were no statistically significant difference between the two groups in the success rate,visual acuity improvement rate and complication rate at 12 months after surgery(P>0.05).Conclusion There is no significant difference in reducing intraocular pressure,protecting vision,complications,drug dependence of glaucoma,and the success rate between FP7 and FP8 AGV implantation.Application of FP8 AGV in adult eyes with insufficient subconjunctival space is a safe and effective choice.
