摘要
目的整理分析某院门诊中药饮片处方点评中存在的问题,提出相应对策,提升合理用药水平。方法选取该院2018年1月—12月2 043张门诊中药饮片处方进行点评分析,对不合理处方进行整理总结,制定合理对策。结果不合理处方334张,其中不规范处方302张(90.42%)、不适宜处方32张(9.58%)、超常处方0张。在对处方不合理问题进行分类统计后发现:缺少中医诊断和辨证占46.11%、脚注问题占20.06%、处方药物未按照"君、臣、佐、使"排列占14.37%,为处方不合理的主要原因。结论通过明确责任人、认真执行处方点评制度、提高不合理处方的干预率、加强培训等措施,可使门诊中药处方合理率有效提升,药师的专业技术价值得到体现。
Objective To sort out and analyze the problems existing in the prescription evaluation of traditional Chinese medicine Pieces in outpatient department of a hospital,and put forward corresponding countermeasures to improve the level of rational drug use.Methods The 2043 prescriptions of Chinese herbal medicines in the outpatient department of our hospital from January to December 2018 were selected and analyzed. The irrational prescriptions were sorted out and summarized, and reasonable countermeasures were formulated.Results 334 unreasonable prescriptions,including 302 non-standard prescriptions (90.42%),32 inappropriate prescriptions (9.58%)and 0 supernormalprescriptions. After classifying and counting the unreasonable prescriptions,we found that:Lack of diagnosis and dialectics of traditional Chinese medicine(46.11%),Footnote problems(20.06%),Prescription drugs were not arranged according to "Jun,Chen,Zuo,Shi"(14.37%). Conclusion By defining the responsible person, conscientiously implementing the prescription evaluation system,improving the intervention rate of unreasonable prescriptions and strengthening training,the rational prescription rate of a hospital has been effectively Promoted,and the professional and technical value of pharmacists has been reflected.
