摘要
目的探讨妊娠期糖尿病对新生儿血脂代谢的影响与主要因素。方法选取2016年1月~2018年2月我院收治的妊娠期糖尿病母亲及新生儿各35例作为研究组,另外选择在我院进行体检的正常妊娠孕妇以及新生儿各35例作为对照组,对两组人群的血脂、血糖水平进行检测,对新生儿血脂代谢的影响进行研究。结果研究组脂蛋白a(Lp-a)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)、甘油三酯(TG)、血总胆固醇(TC)、空腹血糖(FBG)含量高于对照组(P<0.05);研究组女新生儿Lp-a含量低于男新生儿(P<0.05);研究组女新生儿LDL-C、HDL-C、TC含量高于男新生儿(P<0.05);研究组女新生儿TG含量与男新生儿比较无明显差异(P>0.05);研究组大于胎龄儿的Lp-a、LDL-C、HDL-C、TG、TC高于适于胎龄儿、小于胎龄儿(P<0.05);多因素Logistic分析结果表明Lp-a、LDL-C、HDL-C、TC、TC是糖尿病的独立危险因素(P<0.05)。结论妊娠期糖尿病期间,孕妇血糖水平越高,新生儿血脂水平越高,孕妇血糖水平会对新生儿血脂代谢产生影响。
Objective To investigate the effect of gestational diabetes mellitus on blood lipid metabolism in neonates and its main factors. Methods A total of 35 mothers with gestational diabetes mellitus and neonates were enrolled as the study group in our hospital from January 2016 to February 2018. The normal pregnant women who underwent physical examination and 35 neonates in our hospital were selected as the control group. The blood lipids and blood glucose levels of the two groups were tested. The effect on neonatal lipid metabolism was studied. Results The lipoprotein a(Lp-a), low density lipoprotein(LDL-C), high density lipoprotein(HDL-C), triglyceride(TG), total cholesterol(TC), fasting blood glucose(FBG) in the study group was higher than that of the control group(P<0.05). In the study group, the content of Lp-a in the female neonates was lower than that in the male neonates(P<0.05);the content of LDL-C, HDL-C and TC in the female neonates was higher than that in the male neonates(P<0.05). There was no significant difference in TG content between male and female neonates in the study group(P>0.05);Lp-a, LDL-C, HDL-C, TG in the large-forgestational-age infants was higher than that in the appropriate-for-gestational-age infants and small-for-gestationalage infants(P<0.05). Multivariate logistic analysis showed that Lp-a, LDL-C, HDL-C, TC and TC were independent risk factors for diabetes(P<0.05). Conclusion During gestational diabetes period, the higher the blood glucose level of pregnant women, the higher the blood lipid level of newborns, and the blood glucose level of pregnant women will affect the metabolism of blood lipids in neonates.
