摘要
在医疗费用迅速增长、民众对提升医保待遇的期望日益增加的背景下,深入考察医保待遇提升对患者医疗服务利用及费用的影响具有重要的现实意义。本文以医疗服务利用的主要人群——老年人作为研究对象,采用高质量医疗消费数据,以上海市城镇职工医保在退休前后住院报销比例的变动作为政策断点,识别了医保报销比例提高对老年人医疗服务利用的影响。结果发现,住院患者自付比例每降低1个百分点,将使住院人次增加约0.355%,医疗服务利用的价格弹性为-0.36;但对次均住院费用无显著影响。这一结论表明,现阶段适度提高老年群体医保待遇,不会带来费用的过度增长。
In China,the health care expenditures have undergone a rapid growth recently and people urge to have more generous health insurance benefits.Against this background,this paper examines the effect of improving the health insurance benefits on the health care utilization and medical expenditure for elderly people.This paper focuses on elderly people who spend most of current health expenditures.We exploit regression discontinuity design at retirement with a reduction in the hospitalization reimbursement ratio for UEBMI in Shanghai to examine its effects on utilization among elderly people.We find that when the out-of-pocket expenditure ratio decreases by 1%,the inpatient admissions increase by about 0.355%,and the days of hospitalization increase by about 0.357%.However,there is no effect on the inpatient spending.This paper indicates that moderate increase in the current health insurance benefits for the elderly won t lead to excessive health expenditure growth.
