摘要
目的探讨促性腺激素释放激素激动剂(GnRH-a)联合反向添加疗法治疗对子宫内膜异位症手术患者的临床疗效、性激素水平和骨密度影响。方法将2017年6月—2018年12月于我院进行手术治疗的60例卵巢子宫内膜异位症手术患者作为研究对象,按治疗方法分为对照组和观察组,每组30例。对照组给予GnRH-a治疗,观察组在对照组治疗基础上联合反向添加疗法进行治疗,比较2组的临床疗效、性激素水平和骨密度水平,记录并比较2组不良反应发生情况。结果观察组临床总有效率显著高于对照组(P<0.05);2组治疗后组雌激素(E2)、卵泡刺激素(FSH)和血清癌抗原125(CA125)水平均较治疗前降低,且观察组E2、FSH水平高于对照组,CA125水平低于对照组(P<0.05),2组骨密度比较差异无统计学意义(P>0.05);观察组潮热出汗发生率低于对照组(P<0.05)。结论 GnRH-a联合反向添加疗法可提高子宫内膜异位症患者的临床疗效和改善其性激素水平,安全性好,可作为子宫内膜异位症患者术后的首选辅助治疗方案之一。
Objective To investigate effects of gonadotrophin releasing hormone-agonists (GnRH-a) combined with reverse additive therapy on clinical efficacy, sex hormone levels and bone density in treatment of patients with endometriosis after surgery. Methods A total of 60 ovarian endometriosis patients undergoing surgery during June 2017 and December 2018 were divided into control group and observation group ( n =30 in each group) according to therapeutic methods. Control group was treated with GnRH-a, while observation group was treated with reverse addition therapy on the basis of treatment for control group. After treatment, clinical efficacy, sex hormone level and bone density were compared, and incidence rates of adverse reactions during the treatment were recorded in two groups. Results The total effective rate in observation group was significantly higher than that in control group ( P <0.05). After treatment, levels of estrin2 (E2), follicle stimulating hormone (FSH) and cancer antigen 125 (CA125) were significantly lower than those before treatment in two groups;in observation group, E2 and FSH levels were significantly higher, while CA125 level was significantly lower than those in control group ( P <0.05), but there was no significant difference in bone density between two groups ( P >0.05). Incidence rate of hectic fever and sweating in observation group was significantly lower than that in control group ( P <0.05). Conclusion GnRH-a combined with reverse addition therapy may improve clinical efficacy and levels of sex hormones in patients with endometriosis with good safety, and therefore it can be used as one of the first assistant choice after surgery for patients with endometriosis in clinic.
