摘要
目的:探讨基于PDCA循环管理体系在促进儿科抗菌药物(Antiseptic drugs,AD)合理应用的效果。方法:利用医院信息系统调取我院儿科2016~2018年收治的7504例住院患者临床资料进行回顾性分析,以2016年5月~2017年5月未实施PDCA循环管理的3718份住院患儿病历资料为干预前组,以2017年6月~2018年6月实施PDCA循环管理后的3786份住院患儿病历资料为干预后组,对住院患儿抗菌药使用指标进行比较分析。结果:实施PDCA循环管理后,儿科住院患者抗菌药物使用率、使用强度、费用占比、特殊抗菌药使用率与干预前比较均明显降低(P<0.05)。药物品种选择不合理、给药时机不合理率、联合用药不合理、剂量不合理、无理由频繁更换等用药不合理率逐渐下降(P<0.05)。结论:儿科在使用抗菌药物时存在一定不合理现象,实施PDCA循环管理能规范医院儿科抗菌药物使用情况,值得临床推广。
Objective:To probe into the effect of PDCA based circular management system in promoting the rational use of antibacterials drugs(AD)in Department of pediatrics.Methods:Clinical data of 7504 cases of hospitalized children and who treated in our hospital of Department of pediatrics from 2016 to 2018 were retrospectively analyzed by using hospital information system.The data of 3718 cases of hospitalized children who did not implemented the PDCA cycle management from May,2016 to May,2017 were selected as the pre-intervention group.3786 cases of hospitalized children who implemented the PDCA cycle management from June,2017 to June,2018 were taken as the post intervention group.Then,the relative indexes of antibacterials drugs use in hospitalized children were compared and analyzed.Results:After the implementation of PDCA cycle management,the usage rate of antibiotics,the usage intensity,the expense ratio and the usage rate of special antibiotics of pediatric hospitalized children were significantly lower than those before intervention(P<0.05).The irrational rate of drug selection,irrational rate of administration,irrational drug use,irrational dose,and no reason for frequent use of drugs were gradually decreased(P<0.05).Conclusion:There is a certain irrational phenomenon in the use of antibiotics in Department of pediatrics,which the implementation of PDCA circulation management can standardize the use of antibiotics in hospital Pediatrics,thus it is worthy to be promoted in the clinical.
