摘要
目的:探讨自拟利水宣肺汤联合孟鲁司特对支气管哮喘患者炎性因子及肺功能指标的影响。方法:将2016年5月～2017年5月我院收治78例支气管哮喘患者分为观察组和对照组,两组均给予常规治疗,包括给予糖皮质激素类药物、清淡饮食及适量体育锻炼等。对照组在此基础上另给予孟鲁司特,观察组在对照组的基础上加用拟利水宣肺汤,均连续进行治疗10周。对比两组临床疗效、治疗前后肺功能指标,炎性因子[基质金属蛋白酶-9(MMP-9)、基质金属蛋白酶抑制剂-1(TIMP-1)、细胞间黏附分子-1(ICAM-1)及血管内皮生长因子(VEGF)],中医证候评分[咳嗽、胸闷、短气喘促及咳痰]。结果:治疗后,观察组有效率高于对照组(P<0.05),同时FEV_1、FVC及PEF均高于对照组(P<0.05),MMP-9、TIMP-1、ICAM-1及VEGF均明显低于对照组,且咳嗽、胸闷、短气喘促及咳痰评分亦均明显低于对照组(P<0.05)。结论:自拟利水宣肺汤联合孟鲁司特可有效改善支气管哮喘患者肺部功能指标及炎性因子水平,值得进行深入研究。
Objective: To investigate the effect of Lishui Xuanfei Decoction combined with Montelukast on inflammatory factors and pulmonary function indexes in patients with bronchial asthma. Methods: From May 2016 to May 2017,78 patients with bronchial asthma treated in our hospital from May 2016 to May 2017 were divided into observation group and controlled group. Both the two groups received routine treatment,including glucocorticoids,light diet and moderate physical exercise. The controlled group was given Montelukast on the basis of conventional treatment,and the observation group was given Self-made Lishui Xuanfei Decoction on the basis of the controlled group. The two groups were treated continuously for 10 weeks. After treatment,the clinical efficacy of the two groups was observed. Before and after treatment,the lung function indexes[forced vital capacity(FVC),forced expiratory volume in 1 second(FEV1) and peak expiratory flow rate(PEF)],inflammatory factor [matrix metalloproteinase-9(MMP-9)),matrix metalloproteinase inhibitor-1(TIMP-1),intercellular adhesion molecule-1(ICAM-1),vascular endothelial growth factor(VEGF)],and TCM syndrome score [cough,chest tightness,short asthma and cough sputum] of the two groups were compared. Results: After treatment,the effective rate of the observation group was significantly higher than that of the controlled group(P<0.05). After treatment,the FVC,FEV1 and PEF of the observation group were significantly higher those of the controlled group(P<0.05). After treatment,MMP-9,TIMP-1,ICAM-1 and VEGF were significantly lower than those of the controlled group. The scores of cough,chest tightness,short asthma and cough in the observation group were also significantly lower than those in the controlled group(P<0.05). Conclusion: Self-made Lishui Xuanfei Decoction combined with montelukast can effectively improve the lung function index and inflammatory factor level in patients with bronchial asthma,which is worthy of further study.
