摘要
目的观察辛芷通窍颗粒联合克拉霉素治疗慢性鼻窦炎患者的临床疗效及不良反应。方法选择近2年在该院检查的慢性鼻窦炎患者112例,按照随机数字法分为对照组和观察组,各56例,对照组采用克拉霉素常规治疗,观察组在克拉霉素常规治疗基础上加用辛芷通窍颗粒进行结合治疗,服药20天后,记录两组治疗前后视觉模拟量表评分(visual analogue scale,VAS)、Lund-Mackay鼻窦CT扫描评分、Lund-Kennedy鼻内镜黏膜形态学评分、临床疗效进行疗效评价及不良反应监测。结果观察组患者治疗后25例基本痊愈,16例显著有效,8例有效,总有效率为87.50%;对照组患者治疗后18例基本痊愈,12例显著有效,10例有效,总有效率为71.43%。两组总有效率之间比较有显著性差异(P<0.05)。两组患者治疗后,VAS评分均较治疗前明显下降,差异有统计学意义(P<0.05),观察组鼻塞、打喷嚏、流涕评分下调幅度高于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后,Lund-Mackay鼻窦CT扫描评分及Lund-Kennedy鼻内镜评分均下降,与治疗前相比具有显著性差异(P<0.05),观察组下调幅度高于对照组,差异具有统计学意义(P<0.05)。在治疗期间,观察组共有7例(12.50%)患者出现过不良反应,其中3例患者出现恶心,1例患者出现消化不良,1例患者出现腹痛,2例患者出现腹泻;对照组共有5例(8.93%)患者出现过不良反应,其中2例患者出现恶心,1例患者出现消化不良,1例患者出现腹痛,1例患者出现腹泻,不良反应无差异。结论辛芷通窍颗粒联合克拉霉素治疗慢性鼻窦炎的疗效良好,药物不良反应发生率无统计学差异。
Objective To observe the clinical efficacy and side effects of Xinzhi Tongqiao granule combined with clarithromycin in the treatment of chronic sinusitis.Methods There were 112 patients with chronic sinusitis who had been examined in the hospital for the past two years who randomly divided into control group and observation group according to the random number method.The control group was treated with clarithromycin for20days.The observation group was treated with Xinzhi Tongqiao Granule combined with clarithromycin for20days.The visual analogue scale(VAS),Lund-Mackay CT scan score,Lund-Kennedy nasal endoscopic mucosa Morphological score,clinical efficacy evaluation and adverse reaction monitoring were recorded.Results After treatment,25patients in observation group were cured,16cases were significant effective,8cases were effective,and the total effective rate was87.50%.After treatment,18cases in control group were cured,12cases were significantly effective,10cases were effective,the total effective rate was71.43%.There was significant difference between the two groups(P<0.05).The VAS score of the two groups was significantly lower than that of the control group(P<0.05).The scores of nasal congestion,sneezing and runny nose were significantly higher than those of the control group(P<0.05),the difference was statistically significant(P<0.05).Lund-Mackay CT scan score and Lund-Kennedy endoscopic score decreased after treatment(P<0.05),the observation group was lower than the control group(P<0.05).During the treatment in the observation group,there were7(12.5%)patients with adverse reactions,including 3 cases of nausea and1case of dyspepsia,1cases of abdominal pain,2case of diarrhea.In the control group,5cases(8.93%)had adverse reactions,including2cases of nausea and1case of dyspepsia,1cases of abdominal pain,1case of diarrhea.Conclusion Xinzhi Tongqiao granule combined with clarithromycin in the treatment of chronic sinusitis was effective and the incidence of adverse drug reactions was low.
