摘要
目的:探讨金嗓散金丸联合开音丸序贯治疗气滞血瘀痰凝型慢性喉炎(CL)的临床疗效及对患者血液流变性、嗓音声学分析结果的影响。方法:采用随机抽签法将确诊为气滞血瘀痰凝型CL的96例患者均分为两组,其中对照组采用常规西药雾化疗法实施治疗,观察组则采用单号日期服用金嗓散结丸、双号日期服用开音丸的纯中药方式进行治疗。治疗前与用药后1个月分别测定两组血液流变学指标全血高低切黏度、还原黏度、血浆黏度、血沉及红细胞压积;采用ATMOS嗓音测试分析系统对两组治疗前与用药后1个月的嗓音声学情况进行评估;对比两组临床疗效。结果:与治疗前相比,两组全血高、低切黏度、还原黏度、血浆黏度、血沉、红细胞压积6项指标均明显下降(P<0.05),最长发音时间明显延长,发音障碍指数明显提高,基频和振幅微扰、噪谐比值均明显降低(P<0.05)。与对照组比,观察组6项血液流变学指标更低,最长发音时间延长更多,发音障碍指数增加幅度更大(P<0.05)。痊愈率及临床总有效率更高(P<0.05),但两组基频和振幅微扰、噪谐比间差异无统计学意义(P>0.05)。结论:金嗓散结丸与开音丸序贯治疗气滞血瘀痰凝型CL,可有效改善患者血液流变性,调节机体血液微循环,患者嗓音声学特征改善明显,临床疗效较好。
Objective:To investigate the clinical efficacy of Jinsang Sanjiewan combined with kaiyinwan sequential treatment of chronic laryngitis(CL)of Qizhi Xueyu Tanning type and its effect on the hemorheology and voice acoustics of patients.Methods:Randomly draw the law will be diagnosed as Qizhi Xueyu Tanning type CL in 96 cases were divided into two groups,the cases of the control group were treated with conventional western medicine atomization therapy,and the cases of the observation group was treated with Jinsang Sanjiewan in single number of days,and kaiyinwan in double date.Before treatment and 1 month after treatment,the hemorheology index of whole blood,the viscosity,plasma viscosity,erythrocyte sedimentation rate and hematocrit of the two groups were respectively measured.ATMOS voice test and analysis system were used to assessed voice acoustics of the two group.The clinical efficacy was compared between two groups.Results:Compared with those before treatment,the indexes of whole blood high and low shear viscosity,reducing viscosity,plasma viscosity,erythrocyte sedimentation rate and hematocrit of the two groups were significantly decreased(P<0.05).The longest pronunciation time was significantly prolonged,the dysphonia index was significantly improved,the fundamental frequency and amplitude perturbation,the noise ratio were significantly lower(P<0.05).Compared with the control group,the observation group had lower hemorheology indexes,the longest time of pronunciation,more increase of dysphagia index(P<0.05),the cure rate and the total effective rate of the clinic were higher(P<0.05).However,there was no significant difference between the two groups in fundamental frequency and amplitude perturbation and noise ratio(P>0.05).Conclusion:Jinsang Sanjiewan and Kaiyinwan sequential treatment of Qizhi Xueyu Tanning CL,can effectively improve the patient's blood rheology,regulate the body's blood microcirculation,the voice of patients improved significantly acoustic characteristics,clinical significant effect.
