摘要
多节段腰椎管狭窄是指腰椎管狭窄的节段在两个或两个以上,多数腰椎管狭窄症的患者往往出现腰椎多个节段的病变。对于多节段腰椎管狭窄的患者,并不是每个狭窄的节段都能引起相应的临床症状,如果手术处理所有的狭窄节段,则创伤太大;如果仅仅处理个别节段,则可能出现减压不彻底的情况。所以,找到责任节段是治疗的关键,也是困扰临床医师的一个难题。目前,对于责任节段判定的方法主要有一般检查、影像学检查及辅助操作检查等。笔者将对上述判定方法的研究进展进行综述。
Multi-segmental lumbar spinal stenosis refers to two or more segments of lumbar spinal stenosis.Most patients with lumbar spinal stenosis often have multiple segmental lesions of the lumbar spine.Not every stenosed segments will cause corresponding symptoms to patients with multi-segmental lumbar spinal stenosis.If surgery is performed on all stenosed segments,the trauma will be serious;but if it is preformed on individual segments,there will be incomplete decompression.Therefore,the key to treatment is to determine the responsible segment,which is also a major problem to plague clinicians.So far,the methods to determine the responsible segment mainly include general inspection,imageology examination and auxiliary operation examination,etc.This paper reviews the progress on the methods used to determine responsible segment mentioned above.
