摘要
随着机体功能衰退,老年人往往同时患有心脑血管系统、消化系统、呼吸系统等多种慢性疾病。对老年慢病患者及高危人群提供系统、全面、全程、连续、主动、综合的用药管理,以达到促进健康,减少药源性损害,延缓病程,减少并发症,降低致残率,提高生活质量,减少住院率,降低医疗费用的目的。
With the decline of body function,the elderly people often suffer from cardiovascular and cerebrovascular diseases,digestive system,respiratory system and other chronic diseases.Providing a systematic and comprehensive,full,continuous,active and comprehensive management for elderly patients with chronic disease and high-risk groups,in order to promote health,reduce drug-induced damage,delaying disease,reduce complications,reduce morbidity,improve the quality of life,reduce hospitalization rate,and it also can reduce medical costs.
