摘要
目的:分析和评价慢性病项目化管理方案对社区高血压与糖尿病的管理成效。方法:从数据来源的社区医生综合平台中获得对高血压和糖尿病管理数据,并进行统计描述分析。统计分析采用SPSS13.0软件。结果:项目化管理实施后高血压和糖尿病建档人数分别为7 427人和1 489人,规范管理率分别为95.66%和91.88%,治疗率分别为77.89%和69.88%,控制率分别为73.28%和68.73%,知晓率分别为97.66%和95.59%。结论:实行项目化管理对控制高血压和糖尿病等慢性病的效果显著,对慢性病社区综合防治具有积极意义。
Objective To assess the effect of project--management model on managing hypertension and diabetes in Jiangshan town. Methods Data about hypertension and diabetes were obtained from the platform of community doctors. All data were analyzed by SPSS13. 0 software. ResultsAfter project--management model was brought into practice,the number of registered cards for hypertension and diabetes were 7427 and 1489,with the percentages of standardized management as 95.660/~ and 91.88~ respectively. Accordingly,for hypertension and diabetes patients, the percentage of receiving treatment were 77.89%o and 69.88% and the percentage of main symptom controlled were 73. 28% and 68. 73%. In the project--management model, there were 97. 66O//oo and 95. 59% patients aware of hypertension and diabetes. Conclusion The project--management model was effective in controlling hypertension and diabetes. And it also showed great significance in comprehensive prevention of chronic diseases in community health service.
