摘要
本文应用图像分析系统对乳腺导管上皮轻、中度非典型增生（轻中度组）２３例、重度非典型增生（重度组）３３例、浸润性导管癌（癌组）４０例和正常乳腺（正常组）２０例进行细胞核ＤＮＡ含量和形态计量学检测。结果：（１）正常组、轻中度组、重度组和癌组四组间细胞核ＤＮＡ含量（ＤＩ：０９８，１１０，１１４和２１５）和核面积（１６９２，２５５６，３０２６和４９７９μｍ２）、周长（１８２２，２２７１，２４６８和３１５９μｍ）、长径（５８２，７０５，７６２和９４８μｍ）、短径（３６０，４４４，４９０和６４１μｍ）和形状因子（０６３，０６４，０６５和０６９）５个核形态计量参数均分别有显著差异（均Ｐ＜００１），两两组间比较亦均有明显差异（均Ｐ＜００５）。（２）正常组细胞ＤＮＡ倍体（Ｃ）主要为２Ｃ，少数为３Ｃ～４Ｃ，无超４Ｃ细胞；轻中度组２Ｃ细胞低于正常组，３Ｃ～４Ｃ细胞高于正常组，无超５Ｃ细胞；重度组３Ｃ～４Ｃ和５Ｃ细胞明显高于轻中度组，而低于癌组，并可见少量超５Ｃ细胞；癌组约半数细胞为３Ｃ～４Ｃ，异倍体（≥５Ｃ）细胞显著增多。（３）细胞核ＤＮＡ含量与核面积之间呈现显著正相关（ｒ＝０?
For the propose of detecting the characters of nuclear DNA content and its morphometry, 20 cases of breast mild-mediate atypical intraductal hyperplasia (MMAIDH), 30 cases of severe atypical intraductal hyperplasia (SAIDH), 40 cases of infiltrating ductal carcinoma (IDCa) and 20 cases of normal breast (NB) were examined by image analysis. Results: (1) There were significant differences found in the nuclear DNA content (DI:0 98,1 10,1 14 and 2 15,P<0 01) and nuclear area (16 92,25 56,30 26 and 49 79 μm 2, P<0 01), perimeter (18 22,22 71,24 68 and 31 59 μm, P<0 01), long diameter (5 82, 7 05, 7 62 and 9 48 μm, P<0 01), short diameter (3 60, 4 44, 4 90 and 6 41 μm, P<0 01) and formity factor (0 63, 0 64, 0 65 and 0 69, P<0 01) among NB, MMAIDH, SAIDH and IDCa. And significant differences were also shown between two by two groups (P<0 05 for all), respectively. (2) The DNA ploid in NB was mainly 2C, some 3C～4C, no ≥4C; the 2C cells in MMAIDH were lower than in NB, 3C～4C were higher than in NB, no ≥5C; the 3C～4C and 5C in SAIDH were higher than in MMAIDH, but lower than in IDCa, and a few >5C; 3C～4C in IDCa were about a half, aneuploid (≥5C) was significantly increased. (3) A significanlty positive correlation was shown between DI and nuclear area (r=0 57, P<0 01). Conclusions: The nuclear DNA content and its morphometry in SAIDH seem to be closely related to IDCa, and they may be regarded as the reference indexes in evaluating the potential power of malignant change of breast SAIDH.
