摘要
目的介绍一种新生儿、婴儿和儿童兼用的周围静脉全营养混合液(totalnutrientadmixture,TNA)配方。方法将葡萄糖、电解质、7%凡命(18-氨基酸)、20%脂肪乳剂和维生素等配制成一种可兼用于新生儿、婴儿和儿童的TNA,脂肪供热占非蛋白热量之45%,含总热量271kJ/100ml,用量按小儿液体生理日需量公式计算。对TNA的理化性状、稳定性和临床应用进行研究。新生儿、婴儿、儿童各10例临床应用一周,观察有关副作用和血浆蛋白变化。结果TNA在22℃室温下放置24h无理化性状变化。临床应用并发轻度静脉炎2例(2/30),血清甘油三酯一过性增高1例(1/30),无其他代谢并发症。血浆前白蛋白明显增高。结论本配方TNA计算简便,应用安全,可用于各年龄组小儿的短期全肠外或部分肠外营养支持治疗。
Objective To introduce a formula of total nutrient admixture(TNA)that can be used in newborns,infants and children for peripheral venous access.Methods TNA was mainly composed of glucose,compound18-amino acid and20%Intralipid.The total calorie in the TNA was271kJ/100ml,of which lipid was45%.The TNA was given according to the daily requirement for water by body weight.The character and stability of TNA were studied.Each10cases of newborns,infants aged1～12months and children aged1～7years received TNA per peripheral venous postoperative for7days.Patients in control group(n=30)received4∶1sol infusion.Results pH,osmotic pressure and the diameter of particle of TNA were no change when it was put in22℃for24hours.Clinical application showed:2cases(2/30)had mild vasculitis and1case(1/30)had mild increase of triglyceride,no other side effects were found.Prealbumin in plasma was improved in TNA group.Conclusion This simplified formula of TNA can be safely used in newborns,infants and children for short-term total or partial peripheral parenteral nutrition.
