摘要
目的:观察真武汤治疗脾肾阳虚型糖尿病肾病(DN)的临床疗效。方法:将140例患者随机分为对照组和观察组,每组各70例。2组均给予西医常规治疗,观察组在此基础上同时服用真武汤,1剂/d,水煎分服。2组均连续治疗8周。比较2组治疗前后空腹血糖(FPG)、24小时尿蛋白定量、尿N-乙酰-β-D-葡萄糖苷酶(NAG)、尿α_1-微球蛋白(α_1-MG)、尿素氮(BUN)、血清肌酐(Scr)、肌酐清除率(Ccr)和血清光抑素C(cystatin C)的变化情况及临床疗效。结果:FPG、24小时尿蛋白定量、NAG、α_1-MG、BUN、Scr、Ccr、cystatin C等指标治疗前后2组组内比较,差异有统计学意义(P<0.05);治疗后组间比较差异也有统计学意义(P<0.05)。总有效率观察组为88.57%,对照组为67.14%,2组比较差异有统计学意义(P<0.05)。结论:真武汤治疗脾肾阳虚型DN临床疗效显著,可降低血糖和尿蛋白含量,改善肾功能。
Objective： To observe curative effects of ZhenWu decoction in treating diabetic nephropathy （DN）of spleen-kidney Yang deficiency pattern. Methods： All 140 patients were randomly separated into the control groupand the observation group, 70 cases each group. Both groups received conventional therapy ofWestern medicine, theobservation group took ZhenWu decoction, one dose each day, water decocted. Both groups were treated for eightconsecutive weeks. Clinical effects, the changes of serum cystatin C, creatinine clearance rate （Ccr）, serum creati-nine （Scr）, blood usea nitrogen （BUN）, urinary 琢1-microglobulin（琢1-MG）, urinary N-acetyl-茁-D-glucosidase （NAG）,24-hour urinary protein quantification and fasting plasma glucose（FPG） of the patients were compared before and af-ter treating between both groups. Results： The difference was statistically significant in the comparisons of FPG,24-hour urinary protein quantification, NAG, 琢1-MG, BUN, Scr, Ccr, Cys C and others within two groups before andafter treating（P〈0.05）; the difference was statistically significant in the comparisons between both groups aftertreating（P〈0.05）. Total effective rate of the observation group was 88.57%, higher than 67.14% of the control group, and the difference was statistically significant（P〈0.05）. Conclusion： ZhenWu decoction is effective in treat-ing DN of spleen-kidney Yang deficiency pattern, it could lower blood glucose, improve renal function and decreasethe contents of urine protein.
