摘要
目的探讨达芬奇机器人辅助下行宫颈癌根治术治疗4例子宫颈鳞癌的应用价值及体会。方法选择2016年11月至2017年4月在中国人民解放军陆军军医大学第三附属医院(野战外科研究所)应用达芬奇机器人手术系统行宫颈癌根治术的4例患者作为研究对象,观察患者手术时间、术中出血量、淋巴结清扫数目、住院时间、胃肠功能平均恢复时间,是否伴有盆、腹腔脏器损伤,标本残端肿瘤细胞残留,术后感染,出血等短期并发症。结果 4例达芬奇机器人辅助下行宫颈癌根治术均成功完成,无中转开腹,且术中、术后均未发现盆、腹腔脏器受损,标本残端未发现肿瘤细胞残留;平均手术时间(300. 48±49. 32) min,术中平均出血量(145. 36±32. 19) m L,平均清扫淋巴结(22. 42±2. 09)枚,术后胃肠功能平均恢复时间(2. 34±0. 71) d,平均住院时间(10. 47±1. 21) d;术后未见感染、出血等短期并发症。结论达芬奇机器人辅助应用于宫颈癌根治术,安全、可行,创伤较小,具有临床应用价值。
Objective To explore the value and experience of Da Vinci robot assisted radical resection of cervical cancer in the treatment of 4 cases of cervical squamous cell carcinoma. Methods From November 2016 to April 2017,4 cases of radical resection of cervical cancer were operated by Da Vinci robotic surgical system in Third Affiliated Hospital of Army Medical University. The operation time,the amount of intraoperative bleeding,the number of lymph node dissection,the time of hospitalization and the recovery time of the gastrointestinal function were observed,and the short-term complications such as pelvic and abdominal viscera injury,residual tumor cells in the specimen,postoperative infection and bleeding were observed. Results 4 cases of Da Vinci robot assisted radical resection of cervical cancer were completed successfully,without conversion to laparotomy. No pelvic and abdominal organs were damaged during and after operation. The residual tumor cells were not found. The mean operation time was( 300. 48 ± 49. 32) min,the average bleeding volume was( 145. 36 ± 32. 19) m L,and the average clearance of lymph nodes was( 22. 42 ± 2. 09),postoperative gastrointestinal function recovery time was( 2. 34 ± 0. 71) d,average length of stay was( 14. 47 ± 1. 21) d; no postoperative infection,bleeding and other short-term complications. Conclusion Da Vinci robot assisted radical resection of cervical cancer is safe and feasible with less trauma,and has clinical application value.
