摘要
为评价自动痔疮套扎术(RPH)联合外痔翼形切除缝合术治疗环状混合痔的临床疗效,将2016年6月至2017年1月于山东省日照市中医医院住院的120例环状混合痔患者,采用单盲法随机分为治疗组和对照组,每组各60例。治疗组采用RPH加外痔翼形切除缝合术治疗,对照组采用传统外剥内扎术治疗。观察对比2组患者术后肛门疼痛、术中术后出血情况、手术时间、愈合时间、创缘水肿、排便困难、排尿障碍、肛门坠胀、术后住院时间,以及术后6个月痊愈率等情况。结果显示,治疗组在术后肛门疼痛、术中出血、创缘水肿、愈合时间等方面均优于对照组(P <0.05),在排便困难、排尿障碍、肛门坠胀等术后并发症方面,差异无统计学意义(P>0.05)。治疗组手术时间(25±5.0) min,术后住院时间(8±1.2) d,均短于对照组的(34±6.0) min和(12±2.5) d, P <0.05。术后6个月随访治愈率治疗组为95.0%(57/60)高于对照组的86.7%(52/60),差异有统计学意义(P <0.05)。结果表明,RPH联合外痔翼形切除缝合术治疗环状混合痔疗效确切,在减少术后并发症的发生等方面比外剥内扎术具有优势。
To evaluate the clinical effect of Ruiyun procedure for hemorrhoids（RPH） plus wing-like resec- tion ＆ suture in the treatment of circumferential mixed hemorrhoids（CMH） ,the 120 cases of CMH admit- ted in author＇s hospital（2016-06--2017-01） were randomly divided into treatment group and control one（ n = 60, for each）, the former received above surgery combined, while the latter received routine external dis- section ＆ internal ligation;both groups＂ following indexes were compared such as postoperative anal pain, intraoperative and postoperative bleeding status,operation time,healing time the status on wound-edge e- dema,on dyschesia,on dysuria,on anal tenesmus sensation,and hospitalization time,as well as on curative rate at 6 months after surgery, etc. As results, in postoperative anal pain, intraoperative bleeding, wound- edge edema healing time,the former was superior to the latter（ P 〈0.05）;in dyschesia, dysuria, and anal tenesmus ere complication,there was no statistical difference（ P 〉0.05） ;in the time for operation,and for postoperative hospitalization the former was shorter than the latter[（25±5.0） min vs （34±6.0） rain, （8± 1.2） d vs （12±2.5） d, P 〈0.05];in the curative rate at 6 months after surgery the former was significant- ly higher than the latter[（57/60） 95.0% vs （52/60） 86.7%, P 〈0.05].Resutls show that RPH plus wing- like resection ＆ suture for external hemorrhoids in surgical treatment of CMH has certain effect, the ad- vantage over routine procedure in reducing complication.
