摘要
目的探讨右美托咪定在局部麻醉下翼状胬肉切除+干细胞移植日间手术中的应用效果。方法选取44例单眼翼状胬肉患者,随机分为A、B两组,每组22例。A组在2%利多卡因局部浸润麻醉前给予静脉注射右美托咪定1. 0μg/kg,B组仅给予2%利多卡因局部浸润麻醉,两组均行翼状胬肉切除+干细胞移植日间手术。观察患者术中低血压、心动过缓、氧饱和度降低发生情况;采用斯皮尔伯格的状态焦虑问卷(SAI)评估患者焦虑恐惧水平,采用数值评定量表(NRS)评价患者舒适度。结果两组患者术中低血压、血氧饱和度下降、心动过缓的发生率比较,差异均无统计学意义(均P> 0. 05)。两组患者术前SAI及NRS评分比较,差异均无统计学意义(均P> 0. 05),A组术后SAI及NRS评分均优于B组(均P <0. 05)。结论术前应用右美托咪定,可提高局部麻醉下行翼状胬肉切除+干细胞移植日间手术患者舒适度,缓解患者焦虑、恐惧感,安全性高。
Objective To investigate the effect of dexmedetomidine applied to pterygium excision and ambulatory surgery of stem cell transplantation under local anesthesia. Methods Forty-four patients with monocular pterygium were randomly divided into group A and group B, 22 cases in each group.Group A was given 2% lidocaine for local anesthesia following intravenous injection of dexmedetomidine （1.0 μg/kg）,while group B only received local anesthesia with 2% lidocaine.All patients in the two groups underwent pterygium excision and ambulatory surgery of stem cell transplantation.The incidence of hypotension,bradycardia and oxygen saturation decline was observed during operation;the level of anxiety and fear was assessed by Spielberg State Anxiety Inventory（SAI） and the comfort of the patients was assessed by Numerical Rating Scale（NRS） in both groups. Results There was no statistically significant difference in the incidence rate of intraoperative hypotension,oxygen saturation decline or bradycardia between the two groups（all P 〉0.05）.No statistically significant difference was found in the preoperative SAI score or NRS score between the two groups（all P 〉0.05）,the postoperative SAI score and NRS score in group A were superior to those in group B（all P 〈0.05）. Conclusion Preoperative administration of dexmedetomidine can improve the patients′ comfort as well as alleviate patients′ anxiety and fear during pterygium excision and ambulatory surgery of stem cell transplantation under local anesthesia,achieving a high safety.
