摘要
目的:探究双相情感障碍(Bipolar Disorder,BD)患者在不同发病时期疼痛共情能力状况。方法:采用经典疼痛共情范式和中文版人际反应指针量表(Chinese version of interpersonal reactivity index,IRI-C)探究躁狂期、抑郁期和缓解期双相情感障碍患者疼痛共情能力。结果:抑郁期、躁狂期和缓解期患者人际反应指针量表观点采择因子、想象力因子和共情性关心因子得分均低于正常人群;抑郁期患者个人痛苦因子得分显著高于躁狂期和缓解期患者。躁狂期和抑郁期患者判断中性图片的正确率显著低于正常人群,躁狂期、抑郁期和缓解期患者所需反应时均显著高于正常人群。抑郁期患者判断中性图片的正确率与想象力因子、共情性关心因子存在正相关,抑郁期患者判断中性图片的正确反应时与病程存在正相关;缓解期患者对中性图片的正确反应时与个人痛苦因子得分存在正相关。结论:躁狂期、抑郁期和缓解期双相情感障碍患者共情能力均受损,但不同时期受损状况不同。当双相情感障碍患者处在抑郁期时,疼痛共情能力的受损情况较为突出;抑郁期双相情感障碍患者疼痛共情能力受损状况受患者病程影响,患病时间越久,疼痛共情能力受损越严重。
Objective： This article aims to explore the ability condition of empathy patients with BD in different Phases. Methods： By adopting the classic model of empathy for pain and IRI-C, this article explored the empathy ability of pa- tients with BD in manic phase, depressive phase and remission phase. Results： In average, patients in all three phases scored lower than general people in perspective-taking factor, fantasy factor and empathy concern factor according to IRI- C; patients with BD in depressive phase scored notably higher in personal distress factor than patients in manic and remis- sion phase. Patients in manic and depressive phase had a notably a lower accuracy than general people when choosing neu- tral pictures, and patients with BD in all three phrases needed notably more time to respond than general people. For pa- tients in depressive phase, when choosing neutral pictures, their accuracy was positively related to imaginative and empathy care factors, and their correct respond time was positively related to the course of BD. For patients in remission phase, their correct respond time in choosing neural pictures was positively related to the score of personal distress factor of IRI-C. Conclusion： Empathy ability is impaired in patients with BD in all three phases. In depressive phase, the empathy for pain of patients with BD is noticeably damaged; and such damage is influenced by the course of patients： the longer illness duration, the severer impairment the patients has.
