摘要
目的探索CT、MRI在弥漫性轴索损伤法医学鉴定中应用价值。方法在2016年10月—2017年10月期间方便选取123例疑似弥漫性轴索损伤患者为试验对象,随后进行MRI、CT检查,且对比两种诊断方式的价值性和诊断正确率。结果 CT+MRI对小脑诊断正确率为90.00%,对脑干诊断正确率为100.00%,对基底节诊断正确率为96.55%,对灰质与白质交界区诊断正确率为96.55%,且对弥漫性轴索损伤特异性、敏感性、误诊率、漏诊率分别为95.65%、90.00%、4.35%、10.00%,总之,MRI诊断正确率高于CT诊断结果 ,CT+MRI诊断正确率高于单独使用MRI、CT诊断正确率(P<0.05),同时CT+MRI对颅内异常表现的正确率诊断正确率高于CT、MRI组(P<0.05)。结论对弥漫性轴索损伤患者实施MRI联合CT检查,能够为法医鉴定提供依据。
Objective To explore the value of CT and MRI in forensic identification of diffuse axonal injury. Methods A total of 123 patients with suspected axonal injury were convenient selected from October, 2016 to October, 2017. MRI and CT were performed, and the value and diagnosis rate of the two diagnostic methods were compared. Results The correct rate of CT+MRI diagnosis of cerebellum was 90.00%, the correct rate of diagnosis of brainstem was 100.00%, the correct rate of diagnosis of basal ganglia was 96.55%, and the correct rate of diagnosis of gray matter and white matter junction was 96.55%. The specificity, sensitivity, misdiagnosis rate and missed diagnosis rate of axonal injury were 95.65%, 90.00%,4.35%, and 10.00%, respectively. In summary, the diagnostic accuracy of MRI was higher than that of CT diagnosis. The diagnostic accuracy of CT+MRI was higher than that of single correct rate of diagnosis using MRI and CT (P〈0.05), and the correct rate of CT+MRI for the accuracy of intracranial abnormalities was higher than that of CT and MRI groups (P〈0.05).Conclusion MRI combined with CT examination in patients with diffuse axonal injury can provide a basis for forensic identification.
