摘要
目的:作者发明一种改良型导下颌向前组合活动矫治器,分析配合固定矫治器矫治Ⅱ类错的疗效。方法:纳入用固定矫治器矫治恒牙期AngleⅡ类、功能性、齿槽性和轻度骨性患者,均角或低角型、无关节弹响患者,共26例。均来源于西安交通大学口腔医院正畸科2010年1月-2018年1月的就诊病例,佩戴改良型导下颌向前组合活动矫治器,治疗前后拍头颅定位侧位片,并用Steiner和Tweed方法分析测量治疗前后X线测量数据,并用SPSS18.0软件配对t检验统计分析。结果:SNB(°)、ANB(°)、SND(°)、Wit,s、CO-GN(mm)、Pog-NB(mm)、U1-NA(°)和L1-NB距(mm)治疗前后比较,有显著统计学差异(P<0.01),面型改变明显、下颌向前显著、覆覆盖正常。结论:采用作者发明的改良型导下颌向前组合活动矫治器配合固定矫治器矫治恒牙期AngleⅡ类、功能性、齿槽性和轻度骨性患者效果良好,导下颌向前显著、矫治器固位良好、小巧舒适、缩短了治疗时间,提高了临床治疗率。
Objective To evaluate the treatment efficacy of a modified forward mandibular appliancefor correcting class II malocclusion,which is removable applianceinvented by the authors. Methods The sample has selected in patients with functional, alveolar, and mild skeletal malocclusion,no joint symptom , which had treated by fixed appliances combined with a modifiedforward mandibular movement appliance, they haveaverage or low angle in mandibular plane and in the permanent dentition. The caseswere derived from 26 patients in Department of orthodontics,Stomatologicalhospital,Xi＇an Jiao Tong University in 2010 -2018. Before and after treatment, the lateral cephalometric radiographs weretaken, and the X-ray measurement valueswere analyzed by Steiner and Tweed methods, and the statistical analysis of t test were made by SPSS18.0 software. Results There are statistically signifcantin（P〈0.01）in SNB,ANB,SND,Wits,CO-GN（mm）,Pog-NB （mm）,U1-NA,and L1-NB（mm）. The facial profle value hasimprovedsignifcantly, overjet and overbite have been reduced. Conclusion It shows that a modifed orthodontic appliance invented by the author combined with fxed appliance has good effects on the patients with Angle II, functional, alveolar and mild skeletal malocclusion during the permanent dentition. The treatment duration hasbeen reduced.
