摘要
目的:观察高压氧治疗高血压脑出血微创术后患者的临床效果。方法:选取行微创血肿清除术的高血压脑出血患者128例,按随机数字表法分为研究组和对照组,每组64例。两组均给予常规康复训练治疗,研究组在此基础上于术后7d采用高压氧治疗,1/d,14d为1个疗程,1个疗程结束后休息2d再进入下一个疗程,共治疗2个疗程。采用格拉斯哥昏迷量表(glasgow coma scale,GCS)、美国国立卫生研究院卒中量表(national institutes of health stroke scale,NIHSS)、简易精神状态评价量表(mini-mental state examination,MMSE)、日常生活能力量表-巴氏指数(barthel index,BI)分别于微创术前及术后各个时段(7d、14d、21d、30d、37d、3个月、6个月)评定患者的意识状态、神经功能、认知功能及生活质量。结果:综合康复治疗后,两组的GCS、MMSE、BI评分均显著增高(P<0.01),NIHSS评分显著降低(P<0.01),且研究组各时间节点的评分均明显优于对照组(P<0.01)。结论:脑出血颅内血肿患者微创清除术后早期高压氧联合综合康复训练治疗可显著改善意识状态、神经功能、认知功能及生活质量,值得临床推广。
Objective: To observe the clinical effect of hyperbaric oxygenation (HBO) in patients with hypertensive intracerebral hemorrhage(HICH) after minimally invasive surgery. Methods:A total of 128 patients with HICH treated with minimally invasive surgery were divided into the research group and the control group according to the randomly number table method, 64 cases in each group. Two groups were given comprehensive treatment of rehabilitation training, and the research group was treated with HBO 7 days after surgery, 1 times a day for 14 days, the second course started after 2 days rest, the treatment lasted for 2 courses. Respectively, patientsr state of consciousness, neurological function, cognitive function and life quality evaluation before minimally invasive surgery and postoperative period (7 d,14 d,21 d,30 d,37 d,3 months and 6 months) were assessed by glasgow coma scale (GCS), the United States national institutes of health stroke scale (NIHSS), mini--mental state examination ( MMSE), barthel index (BI). Results: After comprehensive rehabilitation treatment,GCS,MMSE and BI of the two groups were significantly inereased (P(0.01) and NIHSS was significantly decreased (P(0.01) ,and the improvement degree of each indicator in research group were better than those of control group ( P〈0. 01). Conclusion: Hyperbaric oxygen treatment on intracranial hematoma with cerebral hemorrhage after minimally invasive surgery can significantly improve the state of consciousness, neurologie function, cognitive function and quality of life, deserve the clinical expansion.
