摘要
目的探讨隐匿型冠心病(latent coronary heart disease,LCHD)患者窦性心率震荡(heart rate turbulence,HRT)以及心率变异性(heart rate variability,HRV)的变化。方法选取55例LCHD患者作为LCHD组和32例健康者作为对照组行24 h动态心电图检测,检出HRT的两个指标——震荡发作(TO)和震荡斜率(TS),以及HRV时域指标(SDNN、SDANN、r MSSD、p NN50)和频域指标(TP、LF、HF、LF/HF),并进行分析。结果 LCHD组与对照组比较,HRT和HRV均有明显异常,其中TS值明显低于对照组,而TO值明显高于对照组(P均<0.05)。HRV指标(SDNN、SDANN、r MSSD、p NN50、TP、LF、HF、LF/HF)明显低于对照组(P均<0.05)。结论 LCHD患者存在明显的自主神经功能紊乱。HRT和HRV可能对LCHD的预测和危险评估有重要作用。
Objective To explore the changes of heart rate turbulence（ HRT） and heart rate variability（ HRV） in patients with latent coronary heart disease（ LCHD）.Methods Twenty-four-hour ambulatory electrocardiography testing was performed on 55 LCHD patients（ LCHD group） and 32 healthy people（ control group）.The two indexes of HRT—turbulence onset（ TO） and turbulence slope（ TS）,HRV time domain indexes of SDNN,SDANN,r MSSD and p NN50,and HRV frequency indexes of TP,LF,HF and LF/HF were measured and analyzed.Results HRT and HRV indexes varied significantly between the two groups.The value of TS in LCHD group was significantly lower than that in the control group（ P 0.05） while TO value was higher（ P 0.05）.In LCHD group,the values of HRV indexes including SDNN,SDANN,r MSSD,p NN50,TP,LF,HF and LF/HF were significantly lower than those in the control group（ P 0.05）.Conclusion Autonomic nervous dysfunction can be obviously identified in LCHD patients.HRT and HRV indexes perhaps play important roles in predicting and making risk evaluation of LCHD.
