摘要
目的探讨单髁置换术治疗膝关节前内侧骨性关节炎的短期疗效。方法采用单髁置换术治疗26例膝关节前内侧骨性关节炎患者(27膝)。通过下肢全长X线片、膝关节功能HSS评分、膝关节活动度、疼痛VAS评分进行疗效评价。结果患者均获得随访,时间3～67个月。伤口均一期愈合。未发生假体周围感染、聚乙烯内衬脱位、假体松动、外侧间室骨关节炎加重等并发症。1例出现小腿静脉丛血栓,经过抗凝治疗后血栓消失。1例出现早期功能锻炼时的中度疼痛,3个月后疼痛好转。随访3～6个月时,患肢力线偏移度较术前明显减少(P<0.05),膝关节功能HSS评分以及膝关节活动度较术前明显提高(P<0.05),VAS评分较术前降低(P<0.05)。结论单髁置换术治疗膝关节前内侧骨性关节炎短期疗效良好,具有创伤小、并发症少、患者恢复快等优点。
Objective To investigate the short-term effect of unicompartmental knee arthroplasty for the treatment of anteromedial knee osteoarthritis. Methods The 26 patients（ 27 knees） with anteromedial knee osteoarthritis received unicompartmental knee arthroplasty. Full length X-ray film of lower limbs,HSS knee score,motion range of knee joint and visual pain analogue score were evaluated preoperatively and postoperatively,respectively. Results All patients were followed up for 3 ～ 67 months. All the cases got satisfactory wound healing,no complications such as periprosthetic infection,polyethylene liner dislocation,prosthesis loosening,and aggravation of lateral compartment osteoarthritis were observed. Venous plexus thrombosis of the shank was observed in 1 case and disappeared after anticoagulant therapy. Another case suffered from moderate pain during early functional exercise,but finally got pain relief after 3 months. Mechanical axis deviation,HSS knee score,motion range of knee joint and visual pain analogue score were significantly improved at 3 ～ 6 months of follow-up（ P 0. 05）. Conclusions Unicompartmental knee arthroplasty has good short-term clinical effect on anteromedial knee osteoarthritis. It is a less traumatic surgery with quick recovery and fewer complication incidence.
