摘要
目的探讨Oxford单髁置换术(UKA)治疗膝关节内侧间室骨性关节炎(OA)的近期疗效。方法将21例膝关节内侧间室OA患者(22膝)行内侧间室UKA。测量膝关节活动度(ROM),采用VAS评分评价疼痛情况,采用KSS进行临床评分和功能评分,采用WOMAC对膝关节进行功能评分。结果患者均获得随访,时间6～24个月。患者切口均一期愈合。无感染、深静脉血栓形成、假体松动、对侧间室病变等并发症发生。术前及末次随访时,ROM为99.6°±16.6°、119.2°±10.2°,VAS评分为2.8分±1.0分、0.7分±0.8分,膝关节KSS临床评分为67.1分±6.6分、84.9分±5.2分,KSS功能评分为57.3分±8.1分、87.0分±5.1分,关节WOMAC评分为46.8分±6.3分、12.1分±3.2分,各项指标末次随访与术前比较差异均有统计学意义(P<0.001)。结论 UKA是治疗膝关节内侧间室OA的有效方法,近期可显著改善膝关节功能。
Objective To investigate the short-term clinical effect of Oxford unicompartmental knee arthroplasty（ UKA） in medial compartment osteoarthritis（ OA） treatment. Methods The 21 patients diagnosed with medial compartment OA（ 22 knees） were admitted. UKA was performed on all the patients. The range of motion（ ROM） of knee joint was recorded. VAS,KSS scoring system and WOMAC scoring system were used to evaluate treatment effect on every patient at the time of the preoperative and last follow-up. Results All the patients were followed up for 6 ～ 24 months. Primary healing was obtained in all patients. There were no complications of infection,deep venous thrombosis,prosthetic loosening and contralateral compartment arthritis. Preoperative and last follow-up ROM of knee joint were 99. 6° ± 16. 6° and 119. 2° ± 10. 2°,and VAS were 2. 8 ± 1. 0 and 0. 7 ± 0. 8,respectively. KSS clinical score were 67. 1 ± 6. 6 and 84. 9 ± 5. 2,and KSS functional score were 57. 3 ± 8. 1 and 87. 0 ± 5. 1,respectively. WOMAC score were 46. 8 ± 6. 3 and 12. 1 ± 3. 2. Each index above showed significant difference between preoperative and last follow-up（ P 0. 001）. Conclusions Oxford UKA is an effective method to treat medial compartment OA,and the function of the knee joint can be improved remarkably in the short-term.
