摘要
目的观察胸腺肽α1联合激素治疗急性前葡萄膜炎(AAU)的临床效果,并评价其对相关细胞因子的影响。方法采用前瞻性研究法将2014年1月至2016年12月成都军区总医院眼科收治的75例AAU患者按照随机数字表法随机分为两组,对照组(n=37,53眼)单纯给予糖皮质激素治疗,观察组(n=38,55眼)给予胸腺肽α1联合糖皮质激素治疗,比较两组患者的治疗效果以及治疗前后的细胞因子(血清TNF-α、IFN-γ、IL-10、IL-23)水平的变化及复发率情况。结果观察组的临床总有效率(92.1%)显著高于对照组(78.4%)(P<0.05);两组患者治疗后的血清TNF-α、IFN-γ、IL-10、IL-23水平均较治疗前显著降低(P<0.05),但观察组治疗后的TNF-α、IFN-γ、IL-10、IL-23水平均低于对照组(P<0.05);随访12个月,观察组的复发率(5.7%)低于对照组(24.6%)(P<0.05)。结论胸腺肽α1联合糖皮质激素治疗AAU的临床疗效显著,降低血清细胞因子水平、改善外周血T淋巴细胞亚群是其主要作用机制。
Objective To observe the clinical effect of thymosin alpha 1 combined with hormone in the treatment of acute anterior uveitis（ AAU） and evaluate its effect on related cytokines. Methods 75 cases of AAU patients in the Department of Ophthalmology of General Hospital of Chengdu Military Region from January 2014 to December 2016 were randomly divided into two groups according to the random digital table method. The control group（ n = 37,53 eyes） was treated with glucocorticoid alone. The observation group（ n = 38,55 eyes） was treated with thymosin alpha 1 combined with glucocorticoid,and the two groups were compared. The therapeutic effect of patients and the changes of cytokines（ serum TNF-α,IFN-γ,IL-10 and IL-23） and the recurrence rate before and after treatment were observed. Results The clinical total effective rate（ 92. 1%） of the observation group was significantly higher than that of the control group（ 78. 4%）（ P〈0. 05）. The levels of serum TNF-α,IFN-γ,IL-10 and IL-23 after treatment in the two groups were significantly lower than those before the treatment（ P〈0. 05）,but the average of TNF-α,IFN-γ,IL-10 and IL-23 after treatment in the observation group was lower than that of the control group（ P〈0. 05）.They were followed up for 12 months. The relapse rate in the observation group（ 5. 7%） was lower than that in the control group（ 24. 6%）（ P〈0. 05）. Conclusion The clinical efficacy of thymosin alpha 1 combined with glucocorticoid in the treatment of AAU is significant. Reducing the level of serum cytokines and improving the peripheral blood T lymphocyte subsets are the main mechanisms of action.
