摘要
目的:探讨闭塞性细支气管炎患儿潮气呼吸肺功能的特征分析。方法:选取本院2012年1月-2017年1月收治的闭塞性细支气管炎患儿30例作为研究组,另选取本院同期健康体检儿童30例作为对照组,采用JAEGER公司的Master Screen Pediatric型肺功能仪对两组潮气呼吸流速-容量曲线进行测定,即观察呼吸频率(respiratory ratio,RR)、每公斤潮气量(tida volume,VT)、吸气时间(Ti)、呼气时间(Te)、吸呼比(Ti/Te);达到峰流速的时间与呼气时间的比值,即达峰时间比(atio TPTEF to TE,TPTEF/TE);达到呼气峰流速时呼出的气体容积与呼气容积的比值,即达峰容积比(ratio VPTEF to VE,VPTEF/VE)。测定呼吸气中期流速比值(ratio TEF50%to TIF50%,TEF50%/TIF50%),并采用单阻断法测定呼吸系统阻力(Rrs)、每千克体质量呼吸系统顺应性(Crs SO/kg)。结果:研究组患儿RR、Ti/Te均高于对照组(P<0.05);而研究组VT、Ti、Te、TPTEF/TE、VPTEF/VE、TEF50%/TIF50%均低于于对照组(P<0.05)。结论:闭塞性细支气管炎患儿较正常幼儿,呼吸系统阻力增高,小气道阻塞,呼吸系统顺应性相对降低,应用婴幼儿肺功能评估可在一定程度上为闭塞性细支气管炎患儿的诊断提供参考依据。
Objective:To investigate the characteristics of tidal breathing and lung function in children with bronchiolitis obliterans.Method:A total of 30 children with bronchiolitis obliterans treated in our hospital from January 2012 to January 2017 were selected as the research group.Another 30 healthy children in the same period were selected as the control group.The tidal breathing velocity volume curves of two groups were measured by JAEGER's Master Screen Pediatric lung function instrument,the respiratory rate(RR),and the volume of tidal gas per kilogram(VT),inspiratory time(Ti),expiratory time(Te),expiration ratio(Ti/Te),ratio of peak velocity to expiratory time,peak time ratio(atio TPTEF to TE,TPTEF/TE),The ratio of the volume of exhaled breath to the expiratory volume when the peak expiratory flow rate was reached,the peak volume ratio(ratio VPTEF to VE,VPTEF/VE).The ratio of respiratory gas medium velocity(TEF50%/TIF50%) was measured and the respiratory system resistance(Rrs) and respiratory system compliance(Crs SO/kg) per kilogram body mass were measured by single block method.Result:RR and Ti/Te in the research group were higher than those in the control group(P〈0.05),while VT,Ti,Te,TPTEF/TE,VPTEF/VE and TEF50%/TIF50% in the research group were lower than those in the control group(P〈0.05).Conclusion:Children with bronchiolitis obliterans have higher respiratory resistance,small airway obstruction and lower respiratory compliance.The application of infant pulmonary function assessment can provide reference for the diagnosis of children with bronchiolitis obliterans to a certain extent.
