摘要
目的探讨前列地尔对老年糖尿病肾病(DN)病人氧化应激、炎性因子及血管内皮因子的影响。方法选取我院2016年3月至2017年5月收治的150例老年DN病人,按照随机数字表法分为前列地尔组和常规组各75例,其中常规组进行常规治疗,前列地尓组在常规治疗的基础上加用前列地尔注射液静脉滴注治疗。比较2组治疗前后氧化应激、炎性因子、血管内皮因子及肾功能指标水平的变化。结果治疗前,2组超氧化物歧化酶(SOD)、丙二醛(MDA)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)、一氧化氮(NO)、内皮素-1(ET-1)、血管性血友病因子(v WF)、β2微球蛋白(β2-MG)及24 h尿蛋白排泄率(UAER)水平差异无统计学意义(P>0.05);治疗后前列地尔组MDA、TNF-α、IL-6、hs-CRP、ET-1、v WF、β2-MG及UAER水平均显著低于常规组(P<0.05),SOD、NO水平显著高于常规组,差异有统计学意义(P<0.05)。结论前列地尔治疗老年DN可明显延缓病人氧化应激恶化,降低机体炎症反应,并对血管内皮功能及肾功能有明显的改善作用,值得临床推广。
Objective To investigate the effects of alprostadil on oxidative stress,inflammatory factors and vascular endothelial factors in elderly patients with diabetic nephropathy( DN). Methods A total of 150 elderly patients with DN from March 2016 to May 2017 in our hospital were divided into alprostadil group and routine group according to random number table,with 75 cases in each group. The routine group was given routine treatment,and the alprostadil group was treated with alprostadil based on routine group. The levels of oxidative stress,inflammatory factors,vascular endothelial factors and renal function indexes were compared between the two groups before and after treatment. Results There were no significant differences in the levels of superoxide dismutase( SOD),malondialdehyde( MDA),tumor necrosis factor α( TNF-α); interleukin 6( IL-6),high sensitivity C-reactive protein( hs-CRP),nitric oxide( NO),endothelin 1( ET-1),von willebrand factor( v WF),β2 microglobulin( β2-MG) and 24 h urinary protein excretion rate( UAER) in the two groups before treatment( P〈0. 05). After treatment,The levels of MDA,TNF-α,IL-6,hs-CRP,ET-1,v WF,β2-MG and UAER in alprostadil group were significantly lower than those in routine group( P〈0. 05),and the levels of SOD,NO in alprostadil group were significantly higher than those in routine group( P〈0. 05). Conclusions Alprostadil in the treatment of elderly diabetic nephropathy can significantly improve the level of oxidative stress,reduce the inflammatory response,and improve the vascular endothelial function and renal function. It is worthy of clinical promotion.
