摘要
[目的]观察并分析内镜下射频治疗术后联合埃索美拉唑镁肠溶片对6种不同中医证型胃食管反流病的临床疗效。[方法]将我院消化内科确诊为胃食管反流病的219例住院患者根据《胃食管反流病中医诊疗专家共识意见》分为6种不同证型:(1)肝胃郁热证;(2)胆热犯胃证;(3)气郁痰阻证;(4)瘀血阻络证;(5)中虚气逆证;(6)脾虚湿热证。均予以内镜下射频治疗,术后联合埃索美拉唑镁肠溶片,观察并分析4周后不同证型的临床疗效。[结果]6种不同证型的患者经治疗后,在西医单项反流症状缓解方面,组间比较差异无统计学意义(P>0.05);在内镜下炎症缓解方面,差异有统计学意义(P<0.05),进一步比较得出,肝胃郁热证的疗效明显优于胆热犯胃证、气郁痰阻证及瘀血阻络证,而其余5组间疗效均无统计学差异;在中医证候缓解方面,组间单因素方差分析显示差异无统计学意义(P>0.05)。[结论]典型单项反流症状及中医证候对不同证型的胃食管反流病患者疗效均无差异;而内镜下的炎症缓解对肝胃郁热证患者疗效显著,明显优于胆热犯胃证、气郁痰阻证及瘀血阻络证的患者。
[Objective]To observe and analyze the clinical curative effect of endoscopic radiofrequency treatment combined with esomeprazole magnesium enteric-coated tablets in six different TCM syndromes of gastroesophageal reflux disease.[Methods]Two hundred and nineteen patients diagnosed with gastroesophageal reflux disease under treatment in 211 hospital were collected in this study.According to Consensus Opinion of TCM Diagnosis and Treatment of Gastroesophageal Reflux Disease,it was divided into：（1） ＂ganweiyure＂ syndrome;（2） ＂ danrefanwei＂ syndrome;（3） ＂ qiyutanzu＂ syndrome;（4） ＂ yuxuezuluo＂ syndrome;（5） ＂zhongxuqini＂ syndrome;（6） ＂ pixushire＂ syndrome.All patients were treated with endoscopic radiofrequency treatment combined with esomeprazole magnesium enteric-coated tablets.The clinical efficacy of different syndromes were observed and analyzed after 4 weeks.[Results]There was no significant difference between groups in western medicine single reflux symptoms after treatment.In endoscopic inflammation relieving,the curative effect were difference after treatment among 6 different syndrome types of patients,and ＂ganweiyure＂ syndrome＇s curative effect was better than ＂ danrefanwei＂ syndrome, ＂ qiyutanzu＂ syndrome and ＂yuxuezuluo＂ syndrome.In TCM symptoms,there was no significant difference among the 6 different syndrome types of patients.[Conclusion]Efficacy in different western medicine single reflux symptoms and TCM symptoms showed no difference.In contrast,efficacy of ＂ganweiyure＂ syndrome in endoscopic inflammation was better than the ＂ danrefanwei＂ syndrome, ＂ qiyutanzu＂ syndrome and ＂yuxuezuluo＂ syndrome,and there was no difference in efficacy between other syndromes.
