玻璃体腔注射抗血管内皮生长因子药物治疗脉络膜新生血管疾病的临床研究 被引量:4

Clinical study on intravitreal injection of anti VEGF drugs in the treatment of choroidal neovascularization

摘要 目的探讨玻璃体腔注射抗血管内皮生长因子(VEGF)治疗脉络膜新生血管疾病的效果。方法选取61例(61眼)脉络膜新生血管疾病患者,按照随机数字表法分组,对照组30例采用光动力疗法治疗,观察组31例于对照组基础上实施玻璃体腔注射抗VEGF药物治疗,观察比较两组临床疗效及治疗前后最佳矫正视力(BCVA)、视网膜黄斑中央厚度(CMT)、视野平均敏感度变化情况,并统计两组并发症发生情况。结果观察组治疗总有效率[93.55%(29/31)]高于对照组[70.00%(21/30)],差异有统计学意义(P〈0.05)。治疗后观察组CMT低于对照组,BCYA及30°、10°视野平均敏感度均高于对照组,差异有统计学意义(P〈0.05);观察组并发症发生率为12.90%(4/31),对照组为3.33%(1/30),差异未见统计学意义(P〉0.05)。结论玻璃体腔注射抗VEGF药物治疗脉络膜新生血管疾病,效果显著,安全性高,可改善患者CMT,提高最佳矫正视力及视野平均敏感度。 Objective To investigate the effect of intravitreal injection of anti vascular endothelial growth factor (VEGF) drugs on choroidal neovascularization. Methods A total of 61 patients (61 eyes) with choroidal neovascularization were randomly divided into two groups. The control group (30 cases) received photodynamic therapy, and the observation group (31 cases) received intravitreal injection of anti VEGF drugs on the basis of the control group. The clinical efficacy, the best corrected visual acuity (BCVA), the macular central thickness (CMT) and the visual field average sensitivity of the two groups were observed and compared, and the occurrence of complications in the two groups was counted. Results The total effective rate of the observation group was [93.55% (29/31) ] was higher than that of the control group [70.00% (21/30), P 〈0. 05]. After treatment, the CMT of observation group was lower than that of control group, and the mean sensitivity of BCVA, 30 degree and 10 degree visual field were higher than those of the control group(P 〈 0. 05). The complication rate was 12.90% (4/31) in the observation group and 3.33% (1/30) in the control group, there was insignificant difference between the two groups ( P 〉 0. 05 ). Conclusions Intravitreal injection of anti VEGF drug is effective and safe in the treatment of choroidal neovascularization. It can improve the CMT and the best corrected visual acuity, and the average visual field sensitivity.
