摘要
目的:观察三期辨证中药复方对老年性骨质疏松髋部骨折患者骨代谢的影响。方法:60例老年性骨质疏松髋部骨折患者,随机分为中药复方组30例和对照组30例,按照标准治疗操作流程骨折予以微创固定处理。中药复方组:中药三期辨证,对照组:禁止患者服用其它影响骨代谢的相关药物。记录骨折愈合时间并分别于术前和术后、术后1周、术后2周、术后1个月、术后3个月检测两组患者血中碱性磷酸酶(ALP)、β胶原特殊序列(β-CTx)、N端骨钙素(N-MID)和25羟基维生素D[VD(25-OH)]相关骨代谢指标。结果:两组患者骨折均达到临床愈合,愈合时间比较,差异有统计学意义(P<0.05)。两组中ALP均呈先降低再增高,术后1月、3月时相比较,差异显著(P<0.05);N-MID与β-CTX表现一致,先增高再降低,术后1月、3月时相比较,差异显著(P<0.05);VD(25-OH)则表现为下降趋势,术后2周、1月、3月时相比较,差异显著(P<0.05,P<0.01)。结论:对于老年性骨质疏松髋部骨折患者宜尽早介入抗骨质疏松治疗,三期辨证中药复方对老年性骨质疏松髋部骨折患者骨代谢影响明显,可以抑制骨吸收、促进骨形成、改善患者体质状态。
Objective: To observe effects of Chinese herbal compound based on three periods syndrome differentiation of TCM on bone metabolism markers of patients with senile osteoporosis and hip fracture. Methods: Sixty patients with senile osteoporosis and hip fracture were randomly divided into the Chinese herbal compound group(30 cases) and the control group(30 cases). The fracture was treated with minimally invasive fixation according to the standard treatment procedure. The patients in the Chinese herbal compound group was treated with three periods syndrome differentiation of TCM, while the control group was prohibited from taking other drugs that affect bone metabolism. The fracture healing time was recorded, and the related bone metabolism indicators, such as ALP, β-CTx, N-MID and VD(25-OH), were detected in two group patients in six periods, before and after operation, postoperative one week, postoperative two week, postoperative one month, postoperative three month respectively. Results: The fractures in both groups reached clinical healing standard, and the difference was statistically significant when comparing the healing time(P〈0.05). The ALP first decreased and then increased in both groups, and the difference was statistically significant between one month and three month at time of postoperation(P〈0.05). VD(25-OH) showed a downward trend, and the difference was statistically significant among one week, one month and two month at time of postoperation(P〈0.05, P〈0.01). Conclusion: For senile osteoporosis patients with hip fractures, it should be involved in anti osteoporosis treatment as soon as possible. Chinese herbal compound based on three periods syndrome differentiation of TCM has significant effects on bone metabolism of patients with senile osteoporosis and hip fracture, inhibiting bone absorption, promoting bone growing, and improving the the patients' constitutions.
