摘要
目的探讨中重度射血分数正常心衰(HFp EF)患者血压变异性(BPV)和N端脑钠肽前体(NT-pro BNP)的关系。方法选取中重度HFp EF患者72例,采用电化学发光免疫法测定其血清NT-pro BNP含量,并用动态血压监测仪获取患者24小时动态血压信息,分析血清NT-pro BNP水平与BPV的关系。结果 (1)不同NT-pro BNP水平组间收缩压(SBP)、舒张压(DBP)和舒张压变异性(DBPV)的差异具有显著的统计学意义(P<0.05),且SBP、DBP、DBPV随着NT-pro BNP水平的升高而降低。(2)NT-pro BNP对数转换值和SBP、DBP、DBPV呈线性负相关。(3)调整性别、年龄、体质指数(BMI)、是否具有高血压病史后,NT-pro BNP转换值和SBP、DBP和DBPV的线性关系依然存在(P<0.05)。结论 DBPV与中重度HFp EF患者NT-pro BNP水平相关,且能在一定程度上反映心衰的状态。
Objective To evaluate the association between the blood pressure variability(BPV) and the plasma N terminal pro B type natriuretic peptide(NT-pro BNP) concentration in moderate and severe heart failure patients with preserved left ventricular ejection fraction(HFp EF). Methods We enrolled a total of 72 moderate and severe heart failure patients who accepted 24-hour ambulatory blood pressure monitoring(ABPM) in Fuwai hospital. The plasma NT-pro BNP concentration was detected by electrochemiluminescence immunoassay. Then the relationship between BPV and plasma NT-pro BNP concentration was analyzed. Results(1) Patients with higher NT-pro BNP level had significant lower level of systolic blood pressure(SBP), diastolic blood pressure(DBP) and diastolic blood pressure variability(DBPV)(P〈0.05).(2) The conversion value of plasma NTpro BNP concentration had significant linear negative correlation to SBP, DBP, and DBPV and the correlation indexes respectively are-0.375,-0.382 and-0.371(P〈0.01).(3) Multiple linear regression analysis showed that the conversion value of plasma NT-pro BNP still significantly and negatively correlated to DBPV after adjusted for sex, age, body mass index, DBP and history of hypertension(P〈0.05). Conclusions There was a negative relationship between DBPV and plasma NT-pro BNP concentration in moderate and severe HFp EF patients, and DBPV may be useful to reflect the condition of heart failure to some extent.
