3.0T高分辨磁共振管壁成像技术评估大脑中动脉粥样硬化性狭窄 被引量:2

摘要 目的探讨3.0T高分辨磁共振管壁成像技术(HR-MRI)在大脑中动脉粥样硬化性狭窄评估中的应用价值。方法选取81例大脑中动脉粥样硬化性狭窄患者,行3.0T HR-MRI扫描,观察斑块位置,测定斑块厚度,计算斑块面积比(斑块面积/斑块所在官腔的截面积)、血管重构率(斑块所在官腔的截面积/近远端正常血管面积平均值)及残腔率(剩余官腔面积/近远端正常血管面积平均值),并分析斑块面积比、血管重构率与残腔率的相关性。结果 81例患者中共检测出大脑中动脉斑块99个,其中环形斑块29个(29.29%),前下壁斑块23个(23.23%),前壁斑块14个(14.14%),前上壁斑块10个(10.10%),下壁斑块9个(9.09%),后壁斑块5个(5.05%),上壁斑块5个(5.05%),下后壁斑块3个(3.03%),后上壁斑块1个(1.01%)。99处斑块厚度为0.3~3.5 mm,平均(1.37±0.49)mm。斑块厚度<0.5 mm者13处(13.13%),斑块厚度≥0.5 mm且≤1.5 mm者72处(72.73%),斑块厚度≥1.5 mm且≤3.5 mm者14处(14.14%)。残腔率与斑块面积比呈负相关(r=-0.938,P=0.001),与血管重构率无关(r=-0.129,P=0.348>0.05)。结论 3.0T HR-MRI可评估大脑中动脉粥样硬化性狭窄斑块分布、血管重构模式,结合3.0T HR-MRI可提高对缺血性卒中的预估,为治疗方案提供指导。
