摘要
目的规范辅助用药的临床使用。方法确定2016年我院销售金额排名前10位的辅助用药,利用循证医学证据逐一建立点评标准,自2017年1月开始进行专项点评及干预。分别选取2016年5-12月(专项点评干预前)与2017年1-8月(专项点评干预后)使用这10种辅助用药的病历各4 000份,比较其点评干预前后的应用情况。结果专项点评干预后,不合理现象比例均明显减少,包括适应证不适宜、溶媒不适宜、联合用药不适宜、用法用量不适宜、重复用药、疗程过长及用药禁忌等,排名前10位的辅助用药占所有辅助用药销售金额的比例由干预前的39.98%降至干预后的31.62%。结论我院初步建立的辅助用药点评方式有效促进了其在临床上的合理使用。
Objective To standardize the clinical use of adjuvant medications. Methods The top 10 adjuvant medications in the list of consumption sum in 2016 were collected and summarized. The criteria were established by use of evidence-based medical evidence,and targeted intervention was implemented from January 2017. Totally 4 000 medical records in each group of the top 10 adjuvant medications were selected from May to December 2016（ pre-special comment group） and from January to August 2017（ post-special comment group）. The application of the top 10 adjuvant medications were compared before and after comments and targeted intervention. Results After special comments and intervention,the ratio of irrational drug use was significantly decreased,including improper indication,inappropriate solvents,improper combination,irrational dosage,repeated drug application,irrational treatment course and drug contraindication. The ratio in sales amount of top 10 adjuvant medications to all adjuvant medications was reduced from 39. 98% to 31. 62%. Conclusion The established comment mode of adjuvant medications in our hospital has effectively improved rational clinical drug use.
