摘要
目的:评价藏药二十五味儿茶凝胶(儿茶凝胶)外治双手背部慢性湿疹的疗效。方法:将2016年1月~2017年3月甘肃中医药大学附属医院收治的42例双手背部慢性湿疹随机分配单手背进行治疗,一只手背采取儿茶凝胶外用,另一只手背应用丁酸氢化可的松乳膏(尤卓尔)治疗。序贯治疗15 d,结束治疗后随访28 d,症状、瘙痒度评分法评价皮损改善情况及瘙痒缓解情况,观察复发情况。结果:儿茶凝胶、尤卓尔有效率分别为85.71%、73.81%,统计无差异(Z=-1.226,P=0.220);两药皆对慢性湿疹红斑,肿胀、鳞屑、苔藓化症状改善显著,但2组间疗效相比无差异(t=-1.463,P=0.147);两药均对瘙痒度缓解显著,但2组间疗效比较无差别(t=-0.735,P=0.465);儿茶凝胶、尤卓尔2组复发率分别为4.76%、21.43%,后者有4例手背呈现色素沉着、毳毛增多现象。结论:儿茶凝胶、尤卓尔均可有效改善慢性湿疹皮损和缓解瘙痒,前者无色素沉着、毳毛增多等副反应、复发率低,可遴选为传统药治疗慢性湿疹的外用新剂型。
Objective： To evaluate the efficacy of Tibetan medicine ershiwuwei catechu gel （catechu gel） in the treatment of chronic eczema of hand back. Methods： 42 cases in Affiliated Hospital of Gansu University of TCM from January 2016 to March 2017 were randomly assigned to be treated with a single hand back , one was treated with Tibetan medicine, catechu gel, and the other was treated with Butyrate Hydrocortisone Cream （Youzhuoer） . The symptoms and itch score were used to evaluate the improvement of skin lesions and itchy relief and to observe the recurrence. Results ： Catechu gel, youzhuoer efficiency had no statistical difference （Z = - 1. 226, P =0. 220）, the effective rate were （85.71%, 73.81% ） ; between the two groups of EASI score after treatment compared with no difference （t = - 1. 463, P = 0. 147）, and both drugs had a significant improvement in the erythema, swelling, scales, and mosses of chronic eczema; there is no difference in itching points （t = -0. 735, P =0. 465）, and two drugs could alleviate the itch- ing significantly; catechu gel and youzhuoer group the recurrence rate （4. 76% , 21.43% ）, the latter had 4 cases with pigmentation and increase of hair growth. Conclusion： Tibetan medicine ershiwuwei catechu gel and Youzhuoer both can effectively improve skin loss and itch of chronic eczema, the former without side effects and lower recurrence rate, which can be selected as traditional medi- cine for the treatment of chronic eczema.
