头部亚低温技术对新生儿缺氧缺血性脑病神经功能修复效果的影响 被引量:2

摘要 目的探讨头部亚低温技术对新生儿缺氧缺血性脑病患儿神经功能修复效果的影响。方法选取2014年5月至2017年5月在我院救治的新生儿缺氧缺血性脑病患儿54例,按随机数字表法分为观察组和对照组各27例。对照组患者接受常规治疗,观察组患者进行常规治疗+头部亚低温技术治疗,观察两组疗效。结果出生14 d、28 d,两组患儿NBNA评分均显著上升,观察组患儿NBNA评分,均显著高于对照组患儿,差异有统计学意义(P<0.05)。观察组患儿临床疗效总有效率92.59%,显著高于对照组患儿62.96%,差异有统计学意义(U=6.968,P<0.05)。治疗后3 d,两组患儿S100-β蛋白和NSE值均呈显著下降,且观察组治疗后3 d和治疗后7 d S100-β蛋白及NSE值显著低于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率为14.81%,对照组为7.41%,差异无统计学意义(P>0.05)。结论头部亚低温技术对新生儿缺氧缺血性脑病患儿神经功能修复效果显著,安全性高。 Objective To investigate the repair effect of head mild hypothermia on neurological function in neonates with hypoxic ischemic encephalopathy. Methods From May 2014 to May 2017,a total of 54 neonates with hypoxic ischemic encephalopathy treated in our hospital were selected as the research objects,and they were randomly divided into control group( 27 cases) and observation group( 27 cases). Both groups were given conventional treatment,in addition,the observation group were given additional head mild hypothermia treatment. The curative effects in the two groups were observed. Results 14 d and 28 d after birth,the NBNA scores in the two groups were significantly higher than those at 7 d after birth,and the differences at different time points within the group were statistically significant( P〈0.05),and the NBNA scores in the observation group at14 d and 28 d after birth were higher than those in the control group( P〈0. 05); the total effective rate in the observation group was 92.59% which was higher than that in the control group( 62.96%)( P〈0.05); 3 d and 7 d after treatment,the S100-β protein and NSE values in the two groups were significantly lower than those before treatment( P〈0.05),and the differences at different time points within the group were statistically significant( P〈0.05),and the S100-β protein and NSE values in the observation group at 3 d and 7 d after treatment were significantly higher than those in the control group( P〈0. 05); the incidence rate of adverse reactions in the observation group was 14.81% which was higher than that in the control group 7.41%,but their difference was not statistically significant( P〈0.05). Conclusions Mild hypothermia technology of head has significant repair effect of neurological function and higher safety for neonates with hypoxic ischemic encephalopathy.
