寒区野外实验性家兔烧冻复合伤的治疗研究

STUDY ON THE TREATMENT FOR EXPERIMENTAL BURN-FREEZE COMBINED INJURY OF RABBIT IN THE FIELD OF COLD REGION

摘要 目的 :探讨以溶栓、扩血管、高压氧、植皮为主的综合性治疗家兔烧冻复合伤的疗效 ,为临床治疗提供依据。方法 :在寒区自然条件下制作烧冻复合伤的模型 ,于伤后 4h按不同的治疗方法分成对照组、常规治疗组和综合治疗组。结果 :对照组及常规治疗组创面水肿分别于伤后 13d治疗后 10d消失 ,两组皮温基本保持在 2 5℃水平 ,常规治疗组 5d后皮片开始有生长 ,2周后 75 %皮片成活。综合治疗组治疗 1d后水肿开始消退 ,7d水肿消失 ,其创面皮温在治疗过程中基本保持在 30℃水平 ,3d后所植皮片生长 ,10d后表皮完全修复。皮片生长速度两组比较有显著差异 (P <0 .0 1)。结论 :烧冻复合伤在积极采用溶栓、扩张血管的基础上 ,再辅助高压氧、植皮等治疗 。 Objective:To observe the therapeutic effect of combined method on burn-freeze combined injury,by dissolving embolus,dilating blood vessel, giving high pressure oxygen and skin graft, so as to provide experimental basis for clinical therapy.Methods:Animal model of burn-freeze combind injury of rabbits in the field of cold region was established. According to different treated methods adopted at 4h after injury, the rabbits were randomly divided into three groups:control group,routine therapy group and combined therapy group.Results:Edema of wound in routine therapy and control groups subsided on 10 d,13 d respectively after injury;the skin temperature of both groups kept at about 25 ℃;the skin graft of wound surface in routine group began to grow on the 5 th day,75% of the skin grafts got growing two weeks later. In combined therapy group, edema began to subside after l d,and completely subsided after 7 d; the skin temperature kept at about 30 ℃ in the course of treatment,skin grafts on wound surface began to grow after 3 d, 100% of the skin grafts got growing after 7 d and the wound healed after 10 d . There was significant difference in growing speed of the skin grafts between the two treated groups(P<0.01).Conclusion:The therapeutic effect on burn-freeze combined injury would be very efficacious after using embolus dissolution, vascular dilation, supplementing with high pressure oxygen and skin graft.
