摘要
目的探讨分析胎心监护对于不典型胎盘早剥早期诊断中的应用价值。方法选择我院2016年1~12月期间收治的不典型胎盘早剥患者160例实施研究,按照早剥面积分组(<1/3者纳入轻型组,≥1/3者纳入重型组),各80例。将两组胎心监护、超声检查和妊娠结局情况进行对比。结果本研究160例患者中,临产前诊断88例(55.00%),产程中诊断45例(28.13%),产后诊断27例(16.88%);在诊断方法方面,胎监+B超共80例(50.00%),单纯胎监74例(46.25%),单纯B超6例(3.75%)。在胎心监护异常患者中,胎心率(FHR)心动过速48例(28.13%),FHR心动过缓43例(26.88%),FHR静止型56例(35.00%),FHR跳跃型16例(20.00%)。在妊娠结局方面,新生儿窒息共42例(26.25%),胎死宫内共30例(18.75%),早产共93例(58.13%),产后出血共32例(20.00%),子宫卒中共17例(10.63%)。结论积极实施胎心监护对于胎盘早剥患者早期诊断具有重要价值,值得推广应用。
Objective To explore and analyze the value of fetal heart monitoring in the early diagnosis of atypical placental abruption. Methods One hundred and sixty patients with atypical placental abruption in our hospital from January 2016 to December 2016 were selected. They were divided into groups according to the abruption area （〈1/3 in mild group and ≥1/3 in severe group）, 80 cases in each group. Fetal heart monitoring, ultrasound examination and pregnancy outcome of two groups were compared. Results Of the 160 patients, 88 cases （55.00%） were diagnosed before parturient, 45 cases （28.13%） were diagnosed in the process of labor and 27 cases （16.88%） were diagnosed after postpartum. In the diagnosis, 80 cases （50.00%） with fetal heart monitoring ＋ B ultrasound, 74 cases （46.25%） with fetal heart monitoring, and 6 cases （3.75%） with B ultrasound. In patients with abnormal fetal heart rate, fetal heart rate （FHR） tachycardia in 48 cases （28.13%）, FHR bradycardia in 43 cases （26.88%）, FHR static type in 56 cases （35.00%）, FHR jumping in 16 cases （20.00%）. In terms of pregnancy outcome, there were 42 cases of neonatal asphyxia （26.25%）, 30 cases of fetal death （18.75%）, 93 cases of premature birth （58.13%）, 32 cases of postpartum hemorrhage （20.00%）, 17 cases of uterine death （10.63%）. Conclusion The positive fetal heart monitoring is of great value in the early diagnosis of placental abruption, and it is worth popularizing.
