摘要
目的探讨半椎体切除术治疗先天性脊柱侧凸的临床效果。方法选择先天性脊柱侧凸患儿106例,分为观察组与对照组各53例,观察组行半椎体切除术治疗,对照组行常规后路松解截骨矫形手术,比较两组手术所需时间、术中出血量、术后住院时间、术后并发症发生情况及手术前后腰椎功能侧凸Cobb角。结果所有患儿手术均顺利完成,术中无严重并发症发生。观察组手术所需时间及术后住院时间均短于对照组,术中出血量也少于对照组(P<0.05);观察组术后3个月并发症发生率低于对照组(P<0.05);术后3个月,两组侧凸Cobb角均小于术前(P<0.05),且观察组小于对照组(P<0.05)。结论相对于松解截骨手术,半椎体切除术治疗先天性脊柱侧凸具有更好的可行性与安全性,能够增强脊柱后凸的矫形效果。
Objective To investigate the clinical effects of hemi-vertebral resection on congenital scoliosis. Methods A total of106 children with congenital scoliosis were selected and were divided into observation group（ n = 53） and control group（ n = 53）. The observation group was treated with hemi-vertebral resection,and the control group with conventional therapy of posterior release and osteotomy.The operation duration,intraoperative blood loss,postoperative hospital stay,incidence of postoperative complications,and preoperative and postoperative Cobb angle of lumbar scoliosis were compared between the two groups. Results The operations were successfully completed among all children,and no severe complications occurred during the operation. The operation duration,postoperative hospital stay and intraoperative blood loss in the observation group were significantly less than those in the control group（ P〈0. 05）. After three month of operation,the incidence rate of complications in the observation group was lower than that in the control group（ P〈0. 05）; the Cobb angles of scoliosis in both groups were significantly smaller compared to the preoperative（ P〈0. 05）,and the angle in the observation group was smaller than that in the control group（ P〈0. 05）. Conclusion Compared to the therapy of posterior release and osteotomy,hemi-vertebral resection achieves better feasibility and safety in the treatment of congenital scoliosis,and it can enhance the efficacy of osteotomy for posterior scoliosis.
