连续性肾脏替代治疗老年顽固性心衰的疗效分析 被引量:3

摘要 目的探究连续性肾脏替代治疗老年顽固性心力衰竭(心衰)的临床疗效。方法 50例老年顽固性心衰患者作为研究对象,所有患者均给予连续性肾脏替代治疗,比较患者治疗前后的心率、血压、经皮血氧饱和度及心功能指标[心脏指数(CI)、射血分数(EF)、心排血量(CO)、脑利钠肽(BNP)]。结果治疗后,患者心率及经皮血氧饱和度分别为(92.0±7.0)次/min、(92.5±0.5)%,优于治疗前的(115.0±13.0)次/min、(83.7±0.1)%,差异有统计学意义(P<0.05);治疗前后收缩压及舒张压比较差异无统计学意义(P>0.05)。治疗后患者的CI、EF、CO、BNP分别为(3.29±0.75)L/(min·m2)、(71.64±6.47)%、(5.73±1.14)L/min、(693.50±452.60)pg/ml,优于治疗前的(1.97±0.48)L/(min·m2)、(41.63±8.45)%、(3.17±0.75)L/min、(2246.20±835.60)pg/ml,差异有统计学意义(P<0.05)。结论连续性肾脏替代治疗老年顽固性心衰能够稳定患者血液动力学,促进患者心功能改善,值得推广应用。 Objective To investigate the clinical efficacy of continuous renal replacement in the treatment of elderly intractable heart failure. Methods A total of 50 elderly patients with intractable heart failure as study subjects all treated with continuous renal replacement, and comparison were made on their heart rate, blood pressure, transcutaneous oxygen saturation and cardiac function index [ cardiac index (CI), ejection fraction (EF), cardiac output (CO) and brain natriuretic peptide (BNP) ] improvement before and after treatment. Results After treatment, patients had heart rate and transeutaneous oxygen saturation respectively as (92.0 ± 7.0) times/min and (92.5 ± 0.5)%, which were all better than (115.0 ± 13.0) times/min and (83.7 ± 0.1)% before treatment, and their difference was statistically significant (P〈0.05). Before treatment, patients had no statistically significant difference in systolic blood pressure and diastolic pressure (P〉0.05). After treatment, patients had CI, EF, CO and BNP respectively as (3.29 ± 0.75) L/(min·m^2), (71.64 ± 6.47)%, (5.73 ± 1.14) L/min and (693.50 ± 452.60) pg/ml, which were all better than (1.97 ± 0.48) L/(min·m2), (41.63 ± 8.45)%, (3.17 ± 0.75) L/min and (2246.20 ± 835.60) pg/ml before treatment, and the difference was statistically significant (P〈0.05). Conclusion Continuous renal replacement for the treatment of elderly intractable heart failure can stabilize the hemodynamics of the patients and promote their improvement of the cardiac function. It is worthy of popularization and application.
