摘要
目的：探讨金樱通淋汤对老年顽固性尿路感染致膀胱颈挛缩患者T淋巴细胞及尿流率影响。方法：研究对象选取74例老年顽固性尿路感染所致膀胱颈挛缩患者,随机分成两组,每组37例。均根据尿细菌培养以及药敏结果选用抗菌药物,对照组予坦索罗辛0.2 mg/次,日1次口服,研究组在上述基础上再予中药金樱通淋汤口服,均治疗15 d。检测外周血T淋巴细胞亚群以及血清球蛋白水平,评估尿动力学变化,行中医主要症状评分,比较临床疗效。结果：与治疗前比较,两组外周血CD＋3、CD4＋以及CD4＋/CD8＋水平升高（P〈0.05）,CD8＋水平降低（P〈0.05）,血清IgG、IgM以及IgA水平升高（P〈0.05）,尿动力学Qmax和Qave升高（P〈0.01）,PVR降低（P〈0.01）,夜尿次数、排尿困难、腰膝酸软以及小腹胀满等中医主要症状评分降低（P〈0.01）;与对照组比较,研究组CD＋3、CD4＋以及CD4＋/CD8＋水平较高（P〈0.05）,CD8＋水平较低（P〈0.05）,IgG、IgM以及IgA水平较高（P〈0.05）,尿动力学Qmax和Qave较高（P〈0.01）,PVR较低（P〈0.01）,夜尿次数、排尿困难、腰膝酸软以及小腹胀满等中医主要症状评分较低（P〈0.01）,临床疗效较高（P〈0.05）。结论：金樱通淋汤能提高顽固性尿路感染所致膀胱颈挛缩机体免疫功能,促进尿液排出。
Objective： To investigate the effect of Jinying Tonglin Decoction in the treatment of bladder neck contracture caused by refractory urinary tract infection in elder patients in T lymphocyte and urinary flow rate. Methods： A total of 74 cases of bladder neck contracture were selected and randomly divided into 2 groups,37 cases in each. Two groups received antibacterials by urine bacterial culture and drug susceptibility. The control group received Tamsulosin 0. 2 mg,once a day. The experiment group received Jinying Tonglin Decoction,and 2 groups were treated for 15 days. The levels of peripheral blood T lymphocyte subsets and serum globulin level,the change of urinary dynamics,the main symptom score of TCM and clinical curative effect were compared. Results： Compared with before treatment,levels of CD＋3,CD4＋ and CD4＋/CD8＋ in peripheral blood in 2 groups increased（ P 〈 0. 05）,levels of CD8＋ decreased（ P 〈 0. 05）,levels of serum IgG,IgM and IgA increased（ P 〈 0. 05）,Qmax and Qave increased（ P 〈 0. 01）,levels of PVR decreased（ P 〈0. 01）,TCM scores of nocturia,dysuria,waist and knees and lower abdominal distension and other major symptoms decreased（ P 〈 0. 01）. Compared with the control group,levels of CD＋3,CD4＋ and CD4＋/CD8＋ in peripheral blood in the experiment group were higher（ P 〈 0. 05）,level of CD8＋ was lower（ P 〈 0. 05）,levels of IgG,IgM and IgA were higher（P 〈 0. 05）,Qmax and Qave were higher（P 〈 0. 01）,levels of PVR were lower（P 〈 0. 01）,TCM scores of nocturia,dysuria,waist and knees and lower abdominal distension and other major symptoms were lower（ P 〈 0. 01）,and the clinical efficacy was higher（ P 〈 0. 05）. Conclusion： Jinying Tonglin Decoction in the treatment of bladder neck contracturecaused by refractory urinary tract infection in elder patients was effective and can improve the immune function and promote urine.
