摘要
目的:探讨影响128层螺旋CT触发扫描腹部实质性脏器门静脉期强化效果的主要因素。方法:选择智能跟踪触发扫描腹部CT增强检查的232例腹部病变患者进行前瞻性研究。扫描触发点阈值设为100 HU,延时20 s、35 s及180 s,分别行动脉期、门静脉期及延迟期扫描,按照对比剂注入流速将患者分为3.0 ml/s组(67例)、3.5 ml/s组(112例)和4 ml/s组(53例)3组;记录3组患者性别、年龄、体重、身高、心率、收缩压、舒张压、触发时间、延迟时间及扫描触发值,测量门静脉期肝脏、脾脏及门静脉主干CT值,运用回归分析、误差条和t检验对影响肝脏、脾脏及门静脉主干强化幅度的主要因素进行判断。结果:流速和性别对门静脉期肝脏、脾脏及门静脉主干强化幅度影响较大;不同流速的3组进行两两比较,流速3.5 ml/s组强化效果优于3 ml/s组,两组肝脏、脾脏及门静脉主干强化幅度比较均有统计学意义(t=1.962,t=2.101,t=2.203;P<0.05);流速3 ml/s与4 ml/s两组比较中门静脉主干强化幅度有统计学意义(t=2.736,P<0.05);不同性别对肝脏、脾脏及门静脉强化效果比较女性强化效果好于男性,其差异均有统计学意义(t=2.452,t=2.096,t=6.057;P<0.05)。结论:128层螺旋CT腹部增强检查选择适当的流速,根据性别适当调整对比剂用量能够提高门静脉期强化效果。
Objective To investigate the main factors that influenced the strengthening effect of 128 slices spiral CT for the portal vein in the abdominal parenchymal organ. Methods: 232 patients with abdominal lesion who received scan of strengthening CT of intelligence tracking trigger were enrolled in the prospective study. The threshold value of scan trigger point was 100 HU, and the delay time of scans at arterial stage, portal vein stage and delay stage were 20s, 35s and 180s, respectively. These patients were divided into 3.0 ml/s group (67 cases), 3.5 ml/s (112 cases) and 4ml/s (53 cases) as the injected flow velocity of contrast agent. The series indicators, included of gender, age, weight, height, heart rate, systolic pressure, diastolic blood pressure, trigger time, delay time and scan trigger value, of the three groups were recorded, respectively. The CT value of liver, spleen and portal vein trunk of portal vein stage were measured. The regression analysis, error strip and t test were applied to judge the main factor that influence strengthening amplitude for liver, spleen and portal vein trunk. Results: The flow velocity and gender have larger influence for liver, spleen and portal vein trunk of portal vein stage. The results of comparison between any two groups indicated that the strengthening effects of 3.5mL/s group for the strengthening amplitude of liver, spleen and portal vein trunk were significantly better than that of 3mL/s group (t=1.962, t=2.101, t=2.203, P〈0.05). The difference of the strengthening amplitude of portal vein trunk between 3mL/s group and 4mL/s group was significantly (t=2.736, P〈0.05). The strengthening effects of female for liver, spleen and portal vein were significantly better than that of male (t=2.452, t=2.096, t=6.057, P〈0.05). Conclusion: The appropriate flow velocity of 128 slices spiral CT and appropriated adjustment of the dosage of contrast agent based on gender can enhance the strengthening effect of portal vein.
