摘要
目的 :从腹腔镜胆囊切除术 (LC)对老年机体的创伤反应程度及术后恢复角度出发 ,探讨合并心肺疾病的老年人行LC的可行性及优势。方法 :将 99例 6 0岁以上同时合并有心肺疾病 (心功能Ⅱ级 )和慢性胆囊疾病的老年病人 ,分为LC组 (49例 ) ,与传统胆囊切除 (OC)组 (5 0例 )。监测术前 1d、术终、术后第 1～ 3天的体温、血糖、血白细胞计数 ,记录切口长度、术中失血量、手术时间、止痛药的使用、恢复饮食的时间、腹腔引流量、术后住院天数及肺部感染等。结果 :LC组创伤反应轻、术后并发症少、恢复快 ,所观察指标均较OC组有显著差异。结论 :从创伤程度及术后恢复角度看 。
Objective:To discuss feasibility and preponderance of laparoscopic cholecystectomy (LC) in geriatric patietns with cardiac and lung disease by studying of wound reaction degree and postoprative recovery after LC and Open Cholecystectomy (OC).Methods:Elderly patients over 6 with chronic bladder-ball diseases and attaching cardiovascular disease and pulmonary disease were divided into two groups:49 cases with LC and 50 cases with OC.Temperature,blood glucose and WBC count were tested before operation?at the end of operation,the first to third day after operation.Incision length?bleeding amount,operation time,use anodyne,time of recovery diet,abdominal drainage amount,postoperational hospitalization day and case of lung inflamation were recorded.Results:All experiment data are significantly different between LC and OC.Conclusions:LC is superior to OC in elderly patients with abnormal cardiac and lung function in wound degree and postoperative recovery.
