摘要
目的探讨程序性死亡分子1(PD1)在类风湿关节炎(RA)患者外周血CD4^+T和CD8^+T细胞上的表达及临床意义。方法应用流式细胞仪检测66例RA患者和30例健康对照者外周血CD4^+T和CD8^+T细胞上PD1表达(平均荧光强度和百分率),比较RA组和健康对照组之间CD4^+T和CD8^+T细胞上PD1表达水平,分析其与RA实验室检查指标的相关性。结果 (1)RA患者CD4^+T、CD8^+T细胞PD1表达的平均荧光强度(MFI)显著高于对照组,差异均有统计学意义(P<0.05);RA患者CD4^+PD1^+T细胞百分率显著高于对照组,差异有统计学意义(P<0.05)。(2)RA患者外周血CD4^+T、CD8^+T细胞PD1表达的MFI与DAS28评分呈正相关性(r=0.49,P<0.000 1;r_s=0.40,P=0.0008)。(3)RA患者外周血CD4^+T细胞PD1表达的MFI与血沉(ESR)、C-反应蛋白(CRP)呈正相关性(r=0.45,P<0.000 1;r=0.39,P=0.001);RA患者外周血CD8^+T细胞PD1表达的MFI与CRP呈正相关性(r_s=0.40,P=0.0009);RA患者外周血CD4^+PD1^+T细胞百分率与CRP呈正相关性(r=0.24,P=0.048)。(4)RA患者外周血CD8^+T细胞PD1表达的MFI与类风湿因子(RF)呈正相关性(r_s=0.32,P=0.009);RA患者外周血CD4^+PD1^+T细胞百分率与RF呈正相关性(r=0.27,P=0.030)。结论 RA患者外周血CD4^+T和CD8^+T细胞PD1表达异常,与疾病的活动性及抗体产生有关。
Objective To investigate the expression of PD1 on CD4^+T and CD8^+T from the patients with rheumatoid arthritis( RA) and to analyze the clinical relevance to disease severity. Methods The expression of PD1 on CD4^+T and CD8^+T were examined from 66 RA patients and 30 healthy controls( HC) by the technique of flow cytometry. The expression of PD1 including mean fluorescence intensity( MFI) on CD4^+T and CD8^+T and percentage of CD4^+PD1^+T and CD8^+PD1^+T were compared between RA patients and HC. Moreover,its correlation with laboratory inspection was analyzed. Results(1) The MFI of PD1 on CD4^+T and CD8^+T from RA patients significantly elevated compared with HC( P〈0. 05),and the percentage of CD4^+PD1^+T from RA patients significantly elevated compared with HC( P〈0. 05).(2) The MFI of PD1 on CD4^+T and CD8^+T in RA patients were positively associated with DAS28( r = 0. 49,P〈0. 000 1;r_s= 0. 40,P = 0. 000 8).(3) The MFI of PD1 on CD4^+T in RA patients were positively associated with CRP and ESR( r = 0. 45,P〈0. 000 1;r = 0. 39,P = 0. 001). The MFI of PD1 on CD8^+T in RA patients was positively associated with CRP( r_s= 0. 40,P = 0. 000 9). The percentage of CD4^+PD1^+T in RA patients was positively associated with CRP( r = 0. 24,P = 0. 048).(4) The MFI of PD1 on CD4^+T in RA patients were positively associated with RF( r_s= 0. 32,P = 0. 009). The percentage of CD4^+PD1^+T in RA patients was positively associated with RF( r = 0. 27,P = 0. 030). Conclusion The aberrations expression of PD1 on CD4^+T and CD8^+T are observed in patients with RA. Increased expression of PD1 on CD4^+T and CD8^+T are correlated with disease activity,and also the production of antibody.
