摘要
原发性胆汁性胆管炎是慢性肝内胆汁淤积性疾病。PBC尚无中医病名,疾病进展中与"风瘙痒""虚病""水臌"等症状相似,推荐以疾病进展阶段的证候特点规范中医病名。通过检索相关文献,发现近年来不断有中医药治疗PBC的临床研究报道。各医家归纳其病机为本虚标实,标实集中在湿、瘀、毒,本虚包括脾虚、肝血虚、肝气虚、肾虚等。中医治疗特色体现在辨证与辨病齐头并进,从本虚出发,提出"扶正可治本""肝气虚"等理论专方专用,分阶段使用中药复方,以及单用中成药治疗,临床研究证实可明显改善患者症状和生化指标,但目前缺乏严谨有效的评价体系以及长期终点事件研究。相关实验研究发现中药有可促进造模小鼠肝细胞增生等作用。
Primary biliary cholangitis is chronic cholestatic liver disease.There is no related traditional Chinese medical name but there are some similar symptoms with the disease such as cutaneous pruritus,deficiency and water tympanites and so on.It is recommended to standardize TCM name with the syndrome characteristics according to stage of the disease.There are increasing TCM clinical trials about treatment of PBC reported in recent years by searching literatures.Pathogenesis is generalized as the root vacuity and tip repletion.Tip repletion includes dampness,poison,blood stasis,and root vacuity includes deficiency in the spleen,liver blood,liver qi and kidney qi.TCM characterizes in both disease cause analysis and differentiation of syndromes.Some characteristics are as followed.The theories such as strengthening right to secure the root,the deficiency of liver qi is put forward.The herbal medicine is phased by stages of disease and Chinese patent medicine is used separately.The clinical trials shows evidence that the symptoms and biochemical indicator is obviously improved,but there is lack of rigorous and effective evaluation system and long-term endpoint study.Related experiments have found that traditional Chinese medicine can promote the proliferation of liver cells in mice.
