318例牙龋病患儿口腔幽门螺旋杆菌感染、唾液口腔pH改变的临床分析 被引量:11

摘要 目的对乳牙龋病患儿口腔幽门螺旋杆菌(helicobacter pylori,HP)感染与唾液口腔pH的改变进行临床分析。方法对2014年8月—2016年8月于石家庄第二医院首次就诊的318例乳牙龋病患儿(观察组)进行口腔幽门螺旋杆菌、唾液pH和内容物检查,与同期因其他疾病就诊的318例非乳牙龋病患儿(对照组)口腔检查结果做为对照,分析2组检查结果。结果观察组龋齿Ⅰ型46.54%,Ⅱ型27.04%,Ⅲ型16.35%,Ⅳ型10.07%;对照组患者HP阳性检出率1.57%,显著低于观察组,观察组Ⅰ型、Ⅱ型、Ⅲ型、Ⅳ型HP阳性检出率和HP-DNA拷贝数依次增高,各类型之间差异具有统计学意义(P<0.05);观察组pH值、IgA、IgG、IgM和溶菌酶含量均显著低于对照组,差异具有统计学意义(P<0.05);观察组pH值、IgA、IgG和IgM含量随着龋齿类型的加重而显著下降(P<0.05),各类型之间差异具有统计学意义(P<0.05)。结论儿童乳牙龋病患儿存在不同程度幽门螺旋杆菌感染,并随着龋病的恶化而加重,口腔内pH的改变和免疫球蛋白水平的下降与HP感染和乳牙龋病的发生有关。 Objective To analyze oral Helicobacter children with caries. Methods A total of 318 children w pyl ith ori (HP) infections and changes of oral salivary pH in 318 caries (observation group, n=318) who were treated in our hospital for the first time between August 2014 and August 2016 underwent examinations of oral Helicobacter pylori salivary pH and contents while the examination results of another 318 children without caries who were treated for other diseases (control group, n=318) were taken as control. The examination results of both groups were analyzed. Results In the observation group, patients with type I caries accounted for 46.54%, type II for 27.04%, type l]I for 16.35% and type IV for 10.07%. The positive HP detection rate was 1.57% in the control group, which was significantly lower than that of the observation group. In the observation group, the positive detection rates of type I, type II, type Ill and type IV HP and copy numbers of HP-DNA increased progressive and levels of IgA, IgM, IgG and lysozyme in the ly, ob and the difference between these types was significant (P〈0.05). pH servation group were significantly lower than those in the control group (P〈0.05), they significantly decreased with the type of caries (P〈0 these types (P〈0.05).
