摘要
目的探讨干燥综合征患者肺功能与血小板活化之间的关系。方法选取干燥综合征患者66例设为观察组,同时选取健康体检者30例设为对照组。采用肺功能仪检测患者肺功能,流式细胞术检测外周血血小板分化抗原CD40配体(CD40L)表达,魏氏法检测血小板(PLT),ELISA试剂盒检测血小板活化因子(PAF)、血小板α颗粒膜糖蛋白-140(GMP-140)、血小板源性生长因子(PDGF)的水平。结果与对照组相比较,观察组肺功能参数明显降低,PLT、PAF、GMP-140、PDGF、CD40L水平明显升高(P<0.05);在观察组患者中,肺功能降低的40例,占所有病例的60.61%,肺功能异常率最高为75%肺活量位的最大呼气流量(FEF_(75)),其次是FEV1/FVC、50%肺活量位的最大呼气流量(FEF_(50))、25%肺活量位的最大呼气流量(FEF_(25))、呼气峰流速值(PEF)。与不伴肺功能降低的SS组比较,伴肺功能降低的观察组的PLT、PAF、PDGF水平明显升高(P<0.05)。相关性分析显示:SS患者FEV1/FVC与PDGF呈负相关,FEF_(25)与CD40L呈负相关(P<0.01),FEF_(50)与PAF、GMP-140呈负相关(P<0.05,P<0.01),FEF_(75)、PEF与PLT呈负相关(P<0.05,P<0.01)。结论干燥综合征患者存在肺功能降低,且肺功能降低与血小板活化及其活化产物的水平密切相关。
Objective To explore the relationship between platelet activation and lung function in patients with sjogren's syndrome. Methods Sixty-six cases of SS and 30 cases of healthy control were selected. The German Jager Master Screen automatic lung function tester was used to detect the pulmonary function parameters. Flow cytometrydetection was used to detect the CD40 L. PLT was analyzed by automatic blood cell analyzer. And the levels of PAF,GMP-140 and PDGF were detected by ELISA. Results Compared to the NC group,pulmonary function parameters of RA were significantly decreased,but PLT,PAF,GMP-140,PDGF and CD40 L were significantly increased( P < 0. 05). Forty patients with abnormal lung function,accounted for 60. 61%,was observed,in which the highest abnormal lung function was FEF_(75),followed by FEV1/FVC,FEF_(50),FEF_(25) and PEF. Compared with SS patients without lung function reduced,PLT,PAF,and PDGF were significantly increased in SS patients with lung function reduced( P < 0. 05). Spearman analysis showed that FEV1/FVC was negatively correlated with PDGF,and FEV25 was negatively correlated with CD40 L( P < 0. 01),FEF_(75) and PEF were negatively correlated with PLT( P < 0. 05). Conclusion There is decreased lung function in patients with SS,which is closely related to platelet activation and products.
