摘要
目的探讨非小细胞肺癌放疗后有症状放射性肺损伤的临床特点。方法随机选取50例行放疗的非小细胞肺癌患者,对其在治疗、随访过程中出现并治疗的有症状放射性肺损伤临床数据进行回顾性分析,并结合并发症常见术语标准展开诊断、分级。结果 50例患者中,2级有症状放射性肺损伤22例(44.0%),3级26例(52.0%),4级0例(0.0%),5级2例(4.0%)。有症状放射性肺损伤(SRILI)是临床多见症状,患者基础肺功能受到影响,包括1s用力呼气体积,用力肺活量,临床表现包括咳嗽、气短、高温,少数患者可见胸部不适;干啰音16例(32.0%),湿啰音8例(16.0%);影像学特征显示放射野内斑片影、条索影、通气之气管征及实变影。18例(36.0%)患者诊断或者分级受到相关因素影响,造成诊断差错,干扰因素分别为合并肺部感染13例(72.2%),合并心衰竭5例(27.8%),合并胸腔积液,且胸腔积液特点无法明确4例(22.2%),记录、检测不详细导致诊断无法有效展开2例(11.1%)。结论有症状放射性肺损伤多为2级、3级,临床特点主要表现为咳嗽、气短、高温、干湿啰音,放射野内斑片影、条索影、通气之气管征及实变影。
Objective To study the clinical characteristics of radiation-induced lung injury in non-small cell lung cancer after radiotherapy.Methods 50 cases of non-small cell lung cancer patients were randomly selected ,the clinical data of symp-tomatic radiation-induced lung injury occurred during radiotherapy and follow-up were retrospectively analyzed ,and combined with the complications common terminology criteria expanded diagnosis ,classification.Results 50 patients,grade 2 had symptoms of radiation-induced lung injury in 22 cases（44.0%）,grade 3 in 26 cases （52.0%）,grade 4 in 0 cases（0.0%）,grade 5 in 2 ca-ses（4.0%）;Symptoms of radioactive respiration lung injury （ SRILI） was common clinical symptoms ,including cough ,shortness of breath,high temperatures,a small number of patients seen chest discomfort;dry rales 16 cases （32.0%）,moist rales 8 cases （16.0%）;imaging features display plaques within the radiation field video ,cable video,ventilation and airway symptoms opaci-ties;18 cases （36.0%） patients diagnosed or classified by the relevant factors ,resulting in diagnostic errors confounders were pulmonary infection in 13 cases （72.2%）,combined heart failure in 5 cases （27.8%）,pleural effusion,pleural effusion,and features not clearly 4 cases （22.2%）,records,testing were not detailed unable to effectively lead to diagnosis in 2 cases （11.1%）.Conclusion Symptomatic radiation-induced lung injury,mostly in grade 2 and 3,clinical features mainly as cough, shortness of breath,airway symptoms of high temperature and dry ,moist rales,the radiation field patchy,cable video,ventilation and opacities .
