摘要
目的观察胰岛素不同给药方式治疗肺结核合并2型糖尿病的临床疗效。方法将80例肺结核合并2型糖尿病患者随机分为对照组与治疗组各40例,对照组采用常规皮下注射胰岛素治疗,治疗组采用胰岛素泵持续皮下注射治疗,治疗后比较临床疗效。结果治疗组糖化血红蛋白、空腹血糖及餐后2h血糖控制情况优于对照组(P<0.05),治疗组痰菌转阴率、病灶有效吸收率及空洞闭合率高于对照组(P<0.05),治疗组血糖达标时间及胰岛素日用量优于对照组(P<0.05),治疗组低血糖发生率低于对照组(P<0.05),治疗组治愈率高于对照组(P<0.05)。结论胰岛素泵持续皮下注射在强化血糖控制的同时有利于肺结核痰菌转阴、病灶吸收及空洞闭合,提高临床疗效,促进肺结核的康复。
Objective To observe the clinical effect of different administration of insulin on pulmonary tuberculosis complicated with type 2 diabetes mellitus. Methods 80 patients with pulmonary tuberculosis complicated with type 2 diabetes were randomly divided into the control group and the treatment group,with 40 cases in each group. The control group was given conventional subcutaneous insulin therapy,while the treatment group was given insulin pump continuous subcutaneous injection treatment. The clinical efficacy was compared after treatment between the two groups. Results The glycosylated hemoglobin,fasting blood glucose and postprandial blood glucose control in the treatment group were better than those in the control group( P < 0. 05). The rate of sputum negative conversion,effective absorption rate and cavity closure rate in the treatment group were higher than those in the control group( P < 0. 05). The incidence of hypoglycemia in the treatment group was lower than that in the control group( P< 0. 05),and the curative ratio in the treatment group was higher than that in the control group( P < 0. 05). Conclusion Intraperitoneal injection of insulin pump can strengthen the control of blood glucose,which is beneficial to the transformation of sputum of tuberculosis,the absorption of the lesion and the closure of the empty cavity,improve the clinical curative effect and promote the rehabilitation of pulmonary tuberculosis.
